Overview
目的利用320排cT-站式成像技术分析颅内高灌注病灶的灌注特征及血管构筑特征,探讨一站式成像技术对颅内病变的临床诊断价值。方法采用320排cT-站式成像技术分析2010年10月-2013年4月成都军区总医院收集的52例颅内高灌注病灶的影像资料,以病变对侧正常脑组织灌注值为对照,分析病灶各灌注参数的y变化特征及血管构筑特征。结果52例中亚急性期脑梗死16例,动静脉畸形9例,血管瘤7例,脑膜瘤12例,胶质瘤8例。所有病例均表现为血容量图(CBV)和(或)血流量图(CBF)的升高,而平均通过时间(MTT)、达峰时间(TTP)、延迟时间(delaytime)等参数变化不一。亚急性脑梗死表现为MTT、TTP、延迟时间延长;动静脉畸形表现为MTT、TTP、延迟时间缩短;血管瘤表现为MTT缩短和TTP、延迟时间延长;脑肿瘤表现为TTP、延迟时间延长,MTT变化不一。血管成像显示所有病例均可见不同程度的血管构筑异常。结论全脑灌注、血管一站式成像分析能够全面反映颅内病灶的灌注特征及血供、微循环变化,有利于指导临床诊治。
