O predomínio das pessoas com demência constitui um problema actual do homem da sociedade, sendo considerado um dos principais problemas de saúde pública com peso social importante, não só pela sua dimensão mas pela forma como afecta aqueles que o rodeiam. A família não deve ser excluída do processo de cuidados, faz parte integrante da vida da pessoa. Compreender as dificuldades expressas por estes familiares no âmbito da gestão do regime terapêutico á pessoa com Doença de Alzheimer e de como vivem esta experiencia do cuidar, foi o mote deste trabalho. A prática baseada na evidência pretende maximizar a qualidade dos cuidados de enfermagem prestados às famílias visando a maior evidência científica. A enfermagem avançada, produtora de resultados sensíveis às pessoas cuidadas, assenta numa reflexão na acção e sobre acção inquirindo: “Quais as dificuldades sentidas pelas famílias, na gestão do regime terapêutico á pessoa com demência (Alzheimer) em contexto domiciliário?”. Foi utilizada a metodologia PICO (Population, Intervention, Comparation e Outcomes). Foram analisados, após um percurso validado, três artigos científicos que emergiram nas bases de dados: CINHAL e MEDLINE. As dificuldades dos familiares cuidadores no âmbito da gestão terapêutica á pessoa com doença de Alzheimer, não é só um problema que se traduz em resultados negativos de saúde, constitui um custo acrescido para as organizações de saúde. Factores individuais, familiares e sociais influenciam o processo de gestão terapêutica de forma negativo ou positiva. Compreender e identificar estes factores precocemente, no sentido de serem controlados aquando da definição das estratégias de cuidar assegura resultados benéficos. Reflectir nesta problemática centrada na família cuidadora ao doente de Alzheimer realça o papel dos enfermeiros de saúde comunitária nas suas funções promotoras de saúde, dinamizadores e interlocutores nas diferentes parcerias com as entidades existentes potenciando os seus recursos. ABSTRACT The prevalence of people with mental diseases is a current problem of mankind in society, being regarded as a major public health problem with considerable social weight, due not only to its size, but also the way it affects those around the patients. The family should not be excluded from the care process, as it forms an integral part of the patient's life. To understand the difficulties expressed by these relatives in the context of management of the therapeutic regime of a patient with Alzheimer’s and how they live this experience of caring was the theme of this work. Evidence-based practice seeks to maximise the quality of nursing care providethe families with a view to greater scientific evidence. Advanced nursing, generating tangible results for patients nursed, is based on reflection in action and on action, asking: “What are the difficulties experienced by the families in the management of the therapeutic regime of a patient with a mental disease (Alzheimer’s) in the context of home care?”. The PICO (Population, Intervention, Comparison and Outcomes) methodology was used. After a validated approach, three scientific articles from the databases CINHAL and MEDLINE were analysed. The difficulties experienced by the relatives who provide care in the context of the therapeutic management of a patient with Alzheimer’s are not merely a problem that yields negative health outcomes, but represents an increased cost for health organisations. Individual, family and social factors influence the therapeutic management process in a beneficial or adverse way. Understanding and identifying these factors at an early stage, in the sense of controlling them upon defining the care strategies, ensures positive results. Reflection on this issue, focused on the family taking care of a patient with Alzheimer’s, highlights the role of community health nurses as health facilitators, triggering and acting as interfaces with the various partnerships with the existing entities, thereby maximising their resources.
