Overview
IntroductionDural arteriovenous fistulas (DAVFs) frequently cause stenosis or occlusion of venous sinuses, but distinguishing between congenital hypoplasia and acquired obstruction remains challenging. Furthermore, no established method exists to assess the feasibility of approaching occluded sinuses for endovascular treatment.Aim of StudyThe purpose of this study was to visualize the lumen and vascular wall characteristics of occluded venous sinuses in patients with intracranial DAVFs using DANTE MRI, and to assess its utility in predicting the feasibility of endovascular access and understanding the nature of the occlusion.MethodWe retrospectively analyzed patients diagnosed with intracranial DAVFs from June 2018 to May 2021 who underwent DANTE-prepared contrast-enhanced MRI before endovascular treatment. Imaging was performed using a 3-T system with a 32-channel head coil. Venous sinus morphology was evaluated, particularly in the cavernous sinus and transverse-sigmoid sinus regions.ResultsDANTE imaging successfully visualized the occluded lumens in cavernous sinus DAVFs, revealing partial patency or thrombus characteristics. In transverse-sigmoid sinus DAVFs, the absence of a discernible lumen suggested congenital hypoplasia. These findings assisted in treatment planning, including the feasibility of transvenous embolization through occluded sinuses in most cases.ConclusionDANTE-prepared MRI enabled visualization of the occluded venous sinus lumen in DAVFs. This technique may aid in selecting optimal therapeutic approaches and enhance understanding of the pathogenesis of venous occlusion. Additionally, detection of stenotic changes and wall thickening could serve as potential markers for predicting disease progression.Conflict of InterestNo
