Overview
According to the report, the incident occurred because the mine operator did not ensure that the dump point could support the weight of the loaded haul truck, conduct an adequate examination to verify that the truck operator dumped material at a safe distance from the edge of the point, or ensure that the truck operator wore a seatbelt. According to MSHA, the incident occurred because the loader operator was moving the loader in a manner that caused the pallet of stone to block his visibility of the staging platform and the miner. According to the report, the incident occurred because the mine operator did not support or otherwise control the rib to protect miners and did not conduct adequate preshift examinations.
