Overview
R245.8; 目的:观察温和灸结合膝关节镜下修复术治疗半月板损伤的疗效,探讨其作用机制.方法:将98例半月板损伤患者采用随机数字表法分为手术组和灸法联合手术组,每组49例.两组均接受膝关节镜下半月板修复术,灸法联合手术组加用温和灸治疗.治疗前后进行Lysholm评分、视觉模拟量表(VAS)评分及患膝关节活动度评定,并测量患膝关节液中转化生长因子(TGF)-β1和血小板源性生长因子(PDGF)水平.术后3个月随访,记录半月板愈合情况.结果:治疗后,两组Lysholm评分均升高,VAS评分均降低,两组组内差异均有统计学意义(P<0.05);灸法联合手术组Lysholm评分高于手术组,VAS评分低于手术组,两项评分的组间差异有统计学意义(P<0.05);两组患膝关节活动度均增加(P<0.05),灸法联合手术组患膝关节活动度大于手术组(P<0.05);与治疗前相比,两组患膝关节液中TGF-β1及PDGF水平均升高(P<0.05),灸法联合手术组患膝关节液中TGF-β1及PDGF水平高于手术组(P<0.05).术后3个月随访,灸法联合手术组的半月板愈合情况明显优于手术组(P<0.05).结论:温和灸联合膝关节镜下半月板修复术治疗半月板损伤在改善膝关节功能,缓解疼痛,增加患膝关节活动度,促进半月板愈合方面的作用优于单独手术治疗,这可能与其上调患者膝关节液中TGF-β1和PDGF水平有关.
