BackgroundNonsteroidal anti-inflammatory drugs (NSAIDs), pancreatic duct stenting, and intensive intravenous hydration have been proven to prevent post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis. Trial participation and guideline changes demanded an assessment of the clinical practice of post-ERCP pancreatitis prophylaxis.AimsThe surveys aim to identify points of improvement to inform and educate ERCPists about current evidence-based practice.MethodsTwo anonymous surveys were conducted among Dutch gastroenterologists in 2013 (n = 408) and 2020 (n = 575) for longitudinal views and attitudes pertaining to post-ERCP pancreatitis prophylaxis and recognition of post-ERCP pancreatitis risk factors.ResultsIn 2013 and 2020, respectively, 121 and 109 ERCPists responded. In the 2013 survey, 98% of them utilized NSAID prophylaxis and 62% pancreatic duct stent prophylaxis in specific cases. In the 2020 survey, the use of NSAIDs (100%), pancreatic duct stents (78%), and intensive intravenous hydration (33%) increased among ERCPists. NSAID prophylaxis was the preferred prophylactic measure for all risk factors in the 2020 survey, except for ampullectomy, pancreatic duct contrast injection, and pancreatic duct cannulation, for which NSAID prophylaxis and pancreatic duct stent combined was equally favored or preferred.ConclusionRectal NSAIDs are the most applied post-ERCP pancreatitis prophylaxis in the Netherlands, followed by pancreatic duct stents and intensive intravenous hydration. Additionally, there is reason to believe that recent guideline updates and active research participation have led to increased prophylaxis implementation.

The Role of Personality in Influencing Work–Family Balance Experience: A Study of Indian Journalists

While an increasing body of research has investigated the situational factors affecting balancing of work and family roles, there is still scarcity of research on the relationship between personal factors and work -family balance (WFB). Therefore, the purpose of this article is to estimate the effect of big five personality traits (openness to experience, conscientiousness, extraversion, agreeableness and neuroticism) on the four dimensions of WFB i.e. work-to-family conflict [WFC], family-to-work conflict [FWC], work-to-family facilitation [WFF] and family-to-work facilitation [FWF] among Indian journalists. From the perspective of WFB, journalists are regarded as an interesting group that has rarely been examined by the scholars. The study validated the ‘fourfold WFB scale and ‘big five personality’ scale in the Indian context. Further, results revealed that big five personality variables emerged as significant predictor set for all the four dimensions of WFB. Unexpectedly, conscientiousness did not reduce WFC, given the unpredictable nature of journalism that pressurizes even a conscientious journalist to sacrifice his family life and work lousy hours. Openness behaved like a double-edged sword that escalate both conflict and facilitation experiences. However, extraversion did not increase facilitation experience for journalists. These findings suggest media organizations to conduct personality test for recruiting suitable candidates, whose personality traits make them best suited to the stressors of journalism profession. Moreover, knowledge of employees’ personality traits may increase the effectiveness of organizational interventions that aims to enhance employees’ WFB.

Background
Post cholecystectomy syndrome is characterized as recurrence of symptoms as experienced before cholecystectomy. In rare cases, a remnant cystic duct is causing these symptoms and occasionally surgical resection is performed. During surgery, visualization of the biliary ducts could be difficult due to inflammation and dense adhesions.
Case presentation
In this article, we presented a 36-year old woman with post-cholecystectomy syndrome in which we evaluated the feasibility of near-infrared (NIR) fluorescence imaging using indocyanine green (ICG) for visualization of the remnant cystic and common bile duct during robot-assisted surgery. Intraoperative visualization of the remnant biliary duct and other important structures was feasible, and resection of the remnant cystic duct was successfully performed under fluorescence guidance, without any complications.
Conclusions
NIR fluorescence imaging of the biliary ducts using ICG does not prolong the operating time, and could potentially decrease the operation time in difficult procedures, because of easy and fast detection of the biliary tract. Furthermore, it is a non-hazardous and non-invasive technique, as it does not require use of radiation and cannot cause bile duct injury. This case illustrated that ICG NIR fluorescence imaging during difficult robot-assisted surgical procedures of the bile ducts is effective and therefore highly recommended.

Domain-specific and nonspecific outcomes of work-family interface

Purpose
The purpose of this paper is to examine the pattern of work-family interface outcomes by empirically testing work-family conflict and facilitation bidirectional dimensions simultaneously in relation to domains-specific (job and family) and domain nonspecific (life) satisfactions. In addition, the indirect effects of work-family interface dimensions on life satisfaction (LS), mediated through both domain-specific satisfactions are also examined to understand which domain satisfaction elicits major impact on LS.
Design/methodology/approach
Structural equation modeling analysis was conducted on questionnaire data obtained from 212 fulltime journalists working in top ten dailies of Punjab, India. Parallel multiple mediated regression was used to estimate specific indirect effects caused by each of the two parallel mediators (job satisfaction (JS) and family satisfaction (FS)).
Findings
Results illustrate that both dimensions of work-family conflict strongly decreased satisfaction of an originating domain than satisfaction of the receiving domain while both dimensions of work-family facilitation increased satisfaction of both the domains on equal basis. Results further reveal that the effect of work-family conflict and facilitation dimensions on LS is indirect rather than direct. On comparison of specific indirect effects results demonstrate that only originating domain satisfaction act as a mediator to work-family conflict and LS relationship, while both JS and FS act as mediators to work-family facilitation and LS relationship.
Practical implications
Media organizations can offer interventions like family friendly policies, overtime pay, more autonomy, work rewards and skill variety, so that employees’ workplace resource reservoir can be strongly built up to meet future work and family demands. In this way, positive intrusion from work-to-family takes place, which leads to more JS and FS and in turn increased overall LS.
Originality/value
The study removes inconsistency regarding pattern of work-family conflict and facilitation outcomes by testing a comprehensive model that integrates originating domain, receiving-domain and domain-nonspecific outcomes.

Journalism has been contented as one of the most unfriendly profession to family life. Given the collectivist nature of the Indian society, attaining work-family balance has become an incessant challenge for journalists. The present paper aims to investigate differences due to gender, life-cycle stages and their interaction terms (gender x life-cycle stages) in the experience of four dimensions of work-family balance, (namely, work-to-family conflict, family-to-work conflict, work-to-family facilitation and family-to-work facilitation). The application of theories like 'Role Strain' and 'Role accumulation to the interplay of work and family roles, triggered bi-directional dimensions of conflict and facilitation, which altogether characterize 'Work-Family Balance' (WFB). Data were collected from 212 fulltime journalists working for the Indian newspaper organizations and analysis was done using two-way multivariate analysis of variance (MANOVA). Results reveal that 'one-size fits all' approach of designing work-family balancing interventions may not prove beneficial because results reveal that women journalists experience more family-to-work conflict in all life stages, less facilitation (both directions) in launching (early-parenthood) life-stage and greater work-to-family facilitation in shifting-gears life-stage (empty-nest) than men journalists. Further, Life-stage differences suggest that journalists during the launching life-stage experience greater family-to-work conflict as compared to the journalists in anticipatory and establishment life-stages. This understanding may be helpful for the media organizations to design necessary practices and policies that can reduce work-family conflict and at the same time, enhance work-family facilitation from the lives of men and women journalists at different stages of life.

Suspected common bile duct stones: reduction of unnecessary ERCP by pre-procedural imaging and timing of ERCP

BackgroundEndoscopic retrograde cholangiopancreatography (ERCP) is the procedure of choice to remove sludge/stones from the common bile duct (CBD). In a small but clinically important proportion of patients with suspected choledocholithiasis ERCP is negative. This is undesirable because of ERCP associated morbidity. We aimed to map the diagnostic pathway leading up to ERCP and evaluate ERCP outcome.MethodsWe established a prospective multicenter cohort of patients with suspected CBD stones. We assessed the determinants that were associated with CBD sludge or stone detection upon ERCP.ResultsWe established a cohort of 707 patients with suspected CBD sludge or stones (62% female, median age 59 years). ERCP was negative for CBD sludge or stones in 155 patients (22%). Patients with positive ERCPs frequently had pre-procedural endoscopic ultrasonography (EUS) or magnetic resonance cholangiopancreatography (MRCP) imaging (44% vs. 35%; P = 0.045). The likelihood of ERCP sludge and stones detection was higher when the time interval between EUS or MRCP and ERCP was less than 2 days (odds ratio 2.35; 95% CI 1.25–4.44; P = 0.008; number needed to harm 7.7).ConclusionsEven in the current era of society guidelines and use of advanced imaging CBD sludge or stones are absent in one out of five ERCPs performed for suspected CBD stones. The proportion of unnecessary ERCPs is lower in case of pre-procedural EUS or MRCP. A shorter time interval between EUS or MRCP increases the yield of ERCP for suspected CBD stones and should, therefore, preferably be performed within 2 days before ERCP.
