Local knowledge, practices, challenges of ethnopharmacologically used medicinal plants in Benin and implications for brain illnesses
Traditional medicine (TM) is a significant resource for primary healthcare management all over the world, and principally in Africa. Quality improvement activities that promote evidence-based practices and the integration of traditional medicine into primary healthcare systems can help improve the quality of patient care. In the Republic of Benin (West Africa), traditional medicine practitioners (TMPs) provide different treatments and ways of use, depending on the ailments and the medicinal plants used. The present study aimed at documenting the knowledge, attitudes and practices of Beninese TMPs regarding the use of medicinal plants and the challenges associated with their activities. A focus group survey was conducted using semi-structured interviews with a sample of 91 TMPs in 8 departments of the Republic of Benin. The respondents had an average age of 50 years old and belonged to various categories of TMPs. Medicinal plants are harvested depending on the season and time of the day, and are dried in the shade before being used as decoctions or infusions. Nevertheless, the majority of TMPs do not conduct the necessary tests for quality control, efficacy or toxicity of the proposed remedies, which raises several scientific interrogations, particularly for the treatment of mental and brain-related disorders. Among ~ 110 plants used in the treatment of several pathologies, 66 were revealed as threatened species. The challenges faced by TMPs are mainly material, financial and technical difficulties. The present study reports the importance of intervention to modernize TM practices in Benin. Quality improvement could enhance healthcare delivery and provide support for evidence-based interventions aimed at addressing behavioral, social, and environmental determinants of health.
Physicochemical Characteristics and Microbiological Quality of Honey Produced in Benin
Honey is a very complex biological product. It has great diversity, giving it a multitude of properties, both nutritionally and therapeutically. This study aimed to study the physicochemical and microbiological characteristics of honeys collected during the dry and rainy seasons in the different phytogeographical areas of Benin. The study revealed that all honeys had pH, water content, electrical conductivity, ash content, free acidity, total sugars, and reducing sugars, respectively, ranging within 3.65–4.09; 12.07–13.16%; 530.25–698.50 μs/cm; 0.42–0.53%; 35.67–40.52 meq/kg; 60–70%; and 58–70%. Moisture content, total sugars, and reducing sugars varied very significantly (p<0.05 to p<0.001) from one area to another and from one season to another. However, only the production season has a significant influence (p<0.05) on the pH of the honey. With regard to the ash content, free acidity, and electrical conduction, no significant difference (p>0.05) between the zones or between the seasons was observed. The results of the microbiological characterization showed that there is heterogeneity in the microbial load. These results have shown that these honeys meet international standards and their characterization will make it possible to obtain Beninese quality labels.
Seroprevalence of Human Immunodeficiency Virus (HIV), Hepatitis B Virus (HBV), and Hepatitis C Virus (HCV) Among Blood Donors in Borgou, Benin in 2023: A Cross-Sectional Study
Blood transfusion remains vital in healthcare but poses risks, particularly from transfusion-transmissible viral infections (TTVIs). This study aims to determine the seroprevalence of HIV, HBV, and HCV among blood donors in Borgou (Benin) in 2023. This prospective, cross-sectional study involved voluntary, non-remunerated blood donors recruited via mobile campaigns and at a fixed site from January to December 2023. Screening for HIV, HBV, and HCV was performed using fourth-generation ELISA (Biorad®). Data analysis used SPSS with Chi-square test of independence (p < 0.05), and multiple logistic regression identified independent risk factors. Among 9646 donors, 87.80% were male (sex ratio 7.19), mostly aged 18–24 (55.93%), with students forming the largest group (58.67%). Mobile units collected 70.80% of donations; 52.60% were repeat donors. Overall TTVI seroprevalence was 9.35%, with HBV (6.29%) most common, followed by HCV (1.78%) and HIV (1.28%). Chi-square tests revealed significant associations between serostatus and donor status, donation site, and occupation, but not sex. Logistic regression identified independent risk factors: age, donor status, and donation site were significantly associated with HIV infection; male sex, older age, occupation, and donor status predicted HBV infection; and only donor status was significantly associated with HCV infection. These findings highlight the need for targeted recruitment and awareness strategies to improve transfusion safety.
Prevalence and determinants of neonatal infections in Benin based on a retrospective study in six reference hospitals
Maternal and neonatal infections pose a significant public health challenge, particularly in developing countries like Benin. This retrospective study investigates the frequency and determinants of maternal and neonatal infections in Benin during 2022, utilizing data from six reference hospitals. The study includes 123 neonates suspected of infection, analyzing factors such as birth weight, breastfeeding practices, clinical delivery parameters, and laboratory-confirmed infection rates. Findings reveal that 32% of suspected cases were confirmed infections, with a higher prevalence among premature newborns and those born in specific hospitals. The study emphasizes the need for improved diagnostic facilities, infection control practices, and awareness among healthcare workers and pregnant women. Recommendations for future research include broader geographic coverage and enhanced training programs.
Metagenomic Analysis of the Abundance and Composition of Antibiotic Resistance Genes in Hospital Wastewater in Benin, Burkina Faso, and Finland
The global emergence and increased spread of antibiotic resistance threaten the effectiveness of antibiotics and, thus, the health of the entire population. Therefore, understanding the resistomes in different geographical locations is crucial in the global fight against the antibiotic resistance crisis. Antibiotic resistance is a global threat to human health, with the most severe effect in low- and middle-income countries. We explored the presence of antibiotic resistance genes (ARGs) in the hospital wastewater (HWW) of nine hospitals in Benin and Burkina Faso, two low-income countries in West Africa, with shotgun metagenomic sequencing. For comparison, we also studied six hospitals in Finland. The highest sum of the relative abundance of ARGs in the 68 HWW samples was detected in Benin and the lowest in Finland. HWW resistomes and mobilomes in Benin and Burkina Faso resembled each other more than those in Finland. Many carbapenemase genes were detected at various abundances, especially in HWW from Burkina Faso and Finland. The bla GES genes, the most widespread carbapenemase gene in the Beninese HWW, were also found in water intended for hand washing and in a puddle at a hospital yard in Benin. mcr genes were detected in the HWW of all three countries, with mcr-5 being the most common mcr gene. These and other mcr genes were observed in very high relative abundances, even in treated wastewater in Burkina Faso and a street gutter in Benin. The results highlight the importance of wastewater treatment, with particular attention to HWW. IMPORTANCE The global emergence and increased spread of antibiotic resistance threaten the effectiveness of antibiotics and, thus, the health of the entire population. Therefore, understanding the resistomes in different geographical locations is crucial in the global fight against the antibiotic resistance crisis. However, this information is scarce in many low- and middle-income countries (LMICs), such as those in West Africa. In this study, we describe the resistomes of hospital wastewater in Benin and Burkina Faso and, as a comparison, Finland. Our results help to understand the hitherto unrevealed resistance in Beninese and Burkinabe hospitals. Furthermore, the results emphasize the importance of wastewater management infrastructure design to minimize exposure events between humans, HWW, and the environment, preventing the circulation of resistant bacteria and ARGs between humans (hospitals and community) and the environment.
Microbiological Safety of Leafy Vegetables Produced at Houeyiho and Sèmè-Kpodji Vegetable Farms in Southern Benin: Risk Factors for Campylobacter spp.
Foodborne infections, mainly those attributable to Campylobacter, are one of the most common causes of intestinal diseases, of bacterial origin in humans. Although the vehicle of transmission is not always identified, the most common vehicles are poultry, poultry products, and contaminated water. In Southern Benin, an excessive use of poultry manure as fertilizer in vegetable farms was noted. This survey aimed to determine the prevalence and concentration of Campylobacter spp., especially Campylobacter jejuni and Campylobacter coli, in selected environmental samples (poultry manure, and irrigation water) and freshly harvested leafy vegetables in two (Houeyiho and Sèmè-Kpodji) vegetable farms in southern Benin. To achieve this objective, we analyzed 280 samples, including 224 samples of leafy vegetables (Solanum macrocarpon and Lactuca sativa capita), 28 samples of irrigation water, and 28 samples of poultry manure. The analysis of the samples taken was carried out according to the modified NF EN ISO 10272-1 standard. Of the 280 samples analyzed in this survey, 63 were positive for Campylobacter contamination. For leafy vegetable samples analyzed in this survey, the contamination rate was of 15.63%. 60.71% of poultry manure samples analyzed were contaminated with Campylobacter spp. and 39.29% of irrigation water samples were contaminated. The statistical analysis of these results showed that there is a correlation between the contamination of leafy vegetables, poultry manure, and irrigations (p<0.01). Campylobacter jejuni (53.97%) was more involved in contaminations than Campylobacter coli (36.57%). This study has shown that there is a real risk of food poisoning by Campylobacter jejuni and Campylobacter coli among consumers of leafy vegetables in southern Benin. The origin of contamination of these leafy vegetables is poultry manure used as fertilizer in vegetable gardens and irrigation water used.
Prevalence of unplanned first pregnancy and socioeconomic factors in Benin: a cross-sectional and analytical study
BackgroundEarly or unplanned pregnancy is an obstacle to the well-being of adolescent girls, especially as early motherhood has health and social consequences on the young mother and the newborn. This also negatively impacts maternal and infant morbidity and mortality. The aim of this study is to determine the prevalence of unplanned first pregnancies and the explanatory factors among adolescent girls.MethodsData were collected from a sample of 738 adolescent girls aged 15–19 years in the 12 departments of Benin. Analyses focused on teenagers who were pregnant for the first time or had at least one child at the time of the survey (337 individuals). Descriptive and bivariate analyses and logistic regression were used.ResultsThe prevalence of unplanned first pregnancies was 80.1% (n=270). The frequency of unplanned first pregnancies was higher among adolescents who had reached secondary school or higher (85.7%, p=0.027), those who had spent their socialisation period in big cities (88.5%, p=0.018) and those whose financial needs were not met (84.8%, p=0.014). The main explanatory factor identified was unmet financial needs. Indeed, adolescents with unmet financial needs were 1.7 times (p=0.041) more likely to have an unplanned first pregnancy than those whose needs were met.ConclusionMultisectoral interventions are needed to reduce the prevalence of unplanned first pregnancies, which can compromise adolescent girls’ academic and career plans. These interventions could focus on sexual and reproductive health education, girl empowerment, improvement of households’ socioeconomic conditions, etc.
Resistance and virulence determinants of faecal Salmonella spp. isolated from slaughter animals in Benin
Objective Salmonella spp. are one of the leading foodborne pathogens worldwide naturally found in the intestines of many animals. People that are in direct contact with the infected animals or their cages may become ill. The aim of this study was to determine the prevalence, antibiogram and virulence genes associated with Salmonella serovars from fecal samples of animals intended for consumption in Southern Benin. Results Out of a total of 406 samples, 2.46% were positive. The isolates identified were multidrug-resistant Salmonella spp. to penicillins, first generation cephalosporins and some aminoglycosides. All Salmonella isolates produced inv A gene of 284 bp, fim A of 85 bp and stn of 260 bp. The spvC gene (571 bp) was present in 10% of the isolates whereas the spvR gene (310 bp) was found in 20% of the isolates. The control strain possessed all the tested genes. The invA gene implies that strains are able to invade epithelial cells. The fimA and stn genes present in all isolates show that they are capable of causing gastrointestinal illness in humans. The presence of spvC and spvR genes suggests the possibility of these strains to produce toxins.
One-Day Prevalence of Extended-Spectrum β-Lactamase (ESBL) and Carbapenemase-Producing Bacteria in Fecal Samples from Surgical Patients: A Concerning Trend of Antibiotic Resistance
Extended-spectrum β-lactamase (ESBL) and carbapenemase producing bacteria are of increasing concern due to their multidrug resistance and infection potential. This study determines the one-day prevalence of faecal carriage of ESBL and carbapenemase producing Gram-negative bacilli. Fecal samples were collected from 30 post-surgery patients (hospitalized for at least 48 hours) in each of the four hospitals involved in the study and were analyzed for antibiotic-resistant bacteria. Identification was done using Maldi Tof mass spectrometry, and antibiotic susceptibility was tested using disk diffusion and specialized tests for ESBL (double disk synergy technique) and carbapenem (NG-TEST CARBA 5) resistance detection. PCR was conducted on isolates to detect betalactam resistance genes, carbapenemase genes and quinolone resistance genes. Out of the 120 patients enrolled, 38.33% (n = 46) and 49.16.33% (n = 59) were found to carry ESBL- and carbapenemase-producing bacteria, respectively, in their fecal samples. Among the isolates, 51.08% (n = 47) exhibited ESBL production, with (44.56%) being the most common species. The identification of bacteria with resistance to carbapenems showed a predominance of the species (44.45%) followed by the species (16.06%) and (13.58%). The study of the association of variables shows a high degree of association (p < 0.05) for the factors independent walking and use of a wheelchair with ESBL production. The most frequently detected genes among ESBL producing bacteria were (91.49%), (70.21%) and (63.82%). (54.68%) was the most detected carbapenemase genes among carbapenemase producing isolates. This study demonstrates, for the first time, a significant prevalence of ESBL and carbapenemase producing gram-negative bacteria among surgical patients in Benin, with multiple resistance genes detected. Findings should be interpreted in light of the cross-sectional design and >48-hour hospitalization criterion.
Antibiotic profiling of multidrug resistant pathogens in one-day-old chicks imported from Belgium to benin
Background Little data exist on the presence of resistant pathogens in day-old chicks imported into Benin. The occurrence of pathogenic bacteria was assessed in 180 one-day-old chicks imported from Belgium and received at the Cardinal Bernardin Gantin International Airport in Cotonou (Benin). The samples included swabbing the blisters of 180 chicks, followed by 18 pools of 10 swabs for bacterial isolation. Classic bacteriological methods based on Gram staining, culture on specific media and biochemical characterization were used. Antibacterial susceptibility screening to antibiotics was conducted using the Kirby–Bauer disc diffusion method, and the results were interpreted according to guidelines from the European Committee on Antimicrobial Susceptibility Testing (EUCAST). DNA extraction was performed by the heat treatment method. Resistance genes were screened by real-time PCR. Results We isolated 32 bacteria, including Escherichia coli (50%), Enterococcus spp. (28%), and coagulase-negative staphylococci (10%). The isolates were investigated for antibiotic resistance against antibiotics using the disk diffusion method and showed that in the Escherichia col i strains isolated, the highest rate of resistance was obtained against ciprofloxacin (81%), followed by trimethoprim + sulfamethoxazole (62%). Enterobacter cloacae was sensitive to all the antibiotics tested. Pseudomonas spp. resistant to amoxicillin and trimethoprim + sulfamethoxazole was noted. The SulII gene was found in all cloacal samples, while the SulI and bla TEM genes were present at 44.44% and 16.67%, respectively. Conclusion This study confirms that imported day-old chicks can be a potential source of dissemination of resistant bacteria in poultry production. A system for immediate detection of resistant bacteria in chicks upon arrival in the country is thus needed.
