Pancreatic cancer incidence and mortality trends: a population-based study
Objectives To analyze trends in pancreatic cancer incidence and mortality in Latin American countries. Methods An ecological study with incidence data from the International Agency for Research on Cancer and mortality data from the World Health Organization. The trend of incidence by Joinpoint regression, the variation of the annual average and the 95% confidence interval were analyzed. Results There were increasing trends in incidence in Brazil, in males, aged 40-59 years, and reduction in Costa Rica. In females, there was stability in all age groups. The mortality rates increased in the elderly in Brazil (AAPC: 1.09%; 95% CI: 0.76; 1.42), Peru (AAPC: 1.76%; 95% CI: 0.36; 3.17) and El Salvador (AAPC: 2.88%; 95% CI: 0.38; 5.43), while in Mexico, there was a reduction. In females, this rate increased in Brazil (AAPC: 1.38%; 95% CI: 1.07; 1.69), Peru (AAPC: 2.25%; 95% CI: 0.68; 3.85), Chile (AAPC: 3.62%; 95% CI:1.96; 5.31), Nicaragua (AAPC: 2.51%; 95% CI: 0.36; 4.71) and Paraguay (AAPC: 1.17%; 95% CI: 0.37; 1.98) and a downward trend was observed in Colombia and Ecuador. Conclusions Pancreatic cancer had a higher incidence in the elderly population of both sexes and an increase of the mortality trend in females was noted.
Age of first pregnancy in Brazil: data from the national health survey
Introduction: The fertility pattern of Brazilian population has changed considerably in the last decades. Socioeconomic and cultural inequalities can influence the age of the first gestation, and the identification of these inequalities is a fundamental aspect of the monitoring and evaluation of women's health care policies. Objective: To analyze the age of women in their first gestation and the related factors in Brazil. Methods: A cross-sectional study using data from the National Health Survey 2013, analyzing women aged 18 to 49 years and their age in the first gestation, categorized by sociodemographic variables. Results: The North Region presented the highest percentage of pregnancy for the stratum of 10 to 14 years. The first pregnancy at the age of 15 to 19 years had the highest percentages for all regions, with a significant difference between the North and Southeast. The Southeast region obtained the highest percentage of the first pregnancy at the age of 30 to 39 years. The first pregnancy at the age of 15 to 19 years was significantly higher among the separated women; without instruction; and who live in rural areas. The first pregnancy of 10 to 14 was associated with the highest number of births that women will have throughout their lives, with a higher prevalence of 5 to 9 births. Conclusions: In Brazil, a large proportion of first pregnancies still occur in adolescence. The states of the northern region stand out with lower average ages in the first pregnancy, and this event is related to the worst socioeconomic conditions. Introdução: O padrão de fecundidade da população brasileira tem se modificado consideravelmente nas últimas décadas. As desigualdades socioeconômicas e culturais podem influenciar a idade da primeira gestação, e a identificação dessas desigualdades é um aspecto fundamental do monitoramento e avaliação das políticas de atenção à saúde das mulheres. Objetivo: Analisar a idade das mulheres em sua primeira gestação e as características socioeconômicas relacionadas no Brasil. Métodos: Estudo seccional que utilizou dados da Pesquisa Nacional de Saúde 2013, analisando mulheres de 18 a 49 anos e sua idade na primeira gestação, categorizada por variáveis socioeconômicas. Resultados: A Região Norte apresentou o maior percentual de gravidez para o estrato de 10 a 14 anos. A primeira gravidez na idade de 15 a 19 anos obteve os percentuais mais elevados para todas as regiões, com diferença significativa entre as regiões Norte e Sudeste. A região Sudeste obteve o maior percentual da primeira gravidez na idade de 30 a 39 anos. A primeira gravidez na idade de 15 a 19 anos foi significativamente maior entre as mulheres separadas; sem instrução; pardas e que residem em área rural. A primeira gravidez de 10 a 14 esteve associado ao maior número de partos que a mulher terá ao longo da vida, com maior prevalência de 5 a 9 partos. Conclusão: no Brasil, uma grande proporção da primeira gestação ainda ocorre na adolescência. Os estados da região norte se destacam com menores médias de idade na primeira gestação, além desse evento estar relacionado às piores condições socioeconômicas.
Testicular Cancer mortality in Brazil: trends and predictions until 2030
Background Despite the fact that testicular cancer presents good prognosis, wide variations in mortality rates have been reported internationally. In Brazil, mortality trends and estimates have not been fully assessed. The objective of the study presented herein is to analyze the mortality trends for testicular cancer in Brazil in the period 2001–2015 and calculate mortality predictions for the period 2016–2030. Methods This is a population-based ecological study that utilized information of the Mortality Information System, on testicular cancer-related deaths in Brazil. Mortality trends were analyzed by Joinpoint regression, and Nordpred was utilized for the calculation of predictions. Results The mortality rate for men, standardized to the world population, varied between 0.36/100,000 for the year 2001, to 0.41/100,000 for the year 2015. There was an increasing trend for Brazil (APC = 1.3% CI95% 0.6; 2.0) and the Southeast region (APC = 1.5% CI95%0.2; 2.7). When analyzing Brazilian data for the period 2016–2030, predictions indicate 2888 deaths due to testicular cancer, which corresponds to a 26.6% change when compared to the 2011–2015 period. This change is mostly explained by an increase in the risk of death (14.2%) when compared with modifications in the demographic structure (12.4%). Conclusions Testicular cancer mortality in Brazil presents increasing trends, and until 2030 these rates continue to increase.
Factors Associated with the Detection of Childhood and Adolescent Cancer in Primary Health Care: A Prospective Cross-Sectional Study
The treatment of childhood cancer has achieved important advances evidencing a significant increase in overall survival; however, the diagnosis of these cases still seems late. Among the main causes for the delay in diagnosis are the issues related to the health system and the first professional who performs the care. The objective of this study is to evaluate the knowledge of primary care physicians and nurses about the most common signs and symptoms of pediatric cancers, as well as the factors related to the obtained scores. This is a prospective cross-sectional study, developed in municipalities in the northeastern region of Brazil. Fifty-one professionals (physicians and nurses) were interviewed through a questionnaire structured as a quiz game about knowledge, training and attitudes regarding the signs and symptoms of childhood cancer. Multiple linear regression analysis was used to evaluate the effect of professional characteristics on the number of correct answers on the implemented questionnaire on knowledge of childhood and adolescent cancer. In the bivariate analysis, the results indicated that the type of employment relationship influences the number of correct answers in the questionnaire used. However, when adjusted for covariates, only the professional category, which means being a medical professional, showed a significant effect on the number of correct answers (β=-7.50, p=0.001). The type of employment relationship of medical professionals and nurses working in primary care had an influence on the number of correct answers for knowledge of childhood and adolescent cancer, but only the professional category (physician) was associated with the highest number when controlled by covariables, thereby justifying the need to improve the curricular training of nurses and greater investments in primary health care for continuing health education that includes infant and adolescent oncology.
Desconhecimento sobre a campanha de vacinação contra o HPV entre estudantes brasileiros: uma análise multinível
Resumo O objetivo deste artigo é analisar a associação entre o desconhecimento sobre a campanha de vacinação contra o HPV entre adolescentes e fatores individuais e contextuais. Estudo transversal com dados da Pesquisa Nacional de Saúde do Escolar (2015). Foi realizada a análise bivariada e calculadas as Razões de Prevalência em uma Regressão de Poisson multinível (IC95%) para verificar o efeito das variáveis no desfecho. O desfecho esteve associado significativamente a ter 15-19 anos de idade (RP=1,36), estudar no turno da tarde/noite (RP=1,05), já ter tido relações sexuais (RP=1,10), com autopercepção do estado de saúde ruim ou muito ruim (RP=1,23), insatisfeito (RP=1,14) ou indiferente (RP=1,15) à sua imagem corporal, que falta às aulas sem consentimento dos pais (RP=1,10) e que estuda em escola pública (RP=1,24). Houve menor prevalência do desfecho entre o sexo feminino (RP=0,24) e em estados com maior desigualdade de renda (RP=0,80). O desconhecimento sobre a campanha de vacinação contra o HPV entre adolescentes foi associado às características individuais e do contexto da escola e da unidade de federação. Esses achados indicam a importância de fortalecer a promoção à saúde voltada aos jovens em vulnerabilidade. It involved a cross-sectional study with data from the National Student Health Survey (2015). Bivariate analysis was performed and Prevalence Ratios were calculated, in a multilevel Poisson Regression (95%CI) to verify the effect of variables on the outcome. The outcome was significantly associated with being 15-19 years old (PR=1.36), studying during the afternoon/night shift (PR=1.05), having already had sexual intercourse (PR=1.10), with self-perceived poor or very poor health status (PR=1.23), dissatisfied (PR=1.14) or indifferent (PR=1.15) regarding their body image, playing truant from classes without parental consent (PR=1.10) and studying at a public school (PR=1.24). There was less prevalence of the outcome among females (PR=0.24) and in states with greater income inequality (PR=0.80). The lack of awareness about the HPV vaccination campaign among adolescents was associated with individual characteristics, the context of the school and the unit of the federation. These findings indicate the importance of enhancing health promotion for vulnerable young people.

O cuidado com os pés e a prevenção da úlcera em pacientes diabéticos no Brasil
Resumo Introdução O Diabetes Mellitus configura-se como um problema de saúde pública em ascensão e, uma vez mal controlado, favorece o desenvolvimento de complicações altamente incapacitantes, como o pé diabético. Objetivo avaliar a prevalência e os fatores associados às ações de prevenção das úlceras dos pés em pacientes diabéticos no Brasil. Método Trata-se de um estudo transversal que utilizou dados da Pesquisa Nacional de Saúde 2013, relativos à proporção de indivíduos com diagnóstico de diabetes em relação às recomendações do cuidado e a prevalência de úlcera nos pés. A PNS sorteou um total de 81.187 domicílios, sendo selecionado um indivíduo por domicílio, sendo entrevistadas 60.202 pessoas. Resultados A realização do exame dos pés em portadores de diabetes por profissionais de saúde foi maior na Região Sudeste e a menor na Região Centro-Oeste. A não realização do exame foi mais prevalente no sexo feminino, em residente em área rural, de cor/raça parda, solteiro, sem instrução ou ensino fundamental incompleto. No Brasil, apenas 58,4% dos pacientes diabéticos que fizeram consulta regular receberam a orientação para examinar os pés; a ocorrência de ferida no pé foi significativamente maior para o sexo masculino; na raça-cor preta/parda; a presença de ferida no pé foi significativamente maior entre os pacientes diabéticos que faziam consulta regularmente. Conclusão Desse modo, no Brasil, a adesão dos profissionais na orientação para prevenção de úlceras diabéticas é considerada precária, com as menores prevalências de realização do cuidado nos grupos mais vulneráveis. Abstract Background Diabetes mellitus is a public health issue and once corrected, it favors the development of highly disabling complications, such as diabetic foot. Objective This study aims to evaluate the prevalence and factors associated with actions to prevent foot ulcers in diabetic patients in Brazil. Method This is a cross-sectional study using data from the National Health Survey 2013, regarding the diagnosis of diabetes related to the recommendations for care and a prevalence of foot ulcers. Results The examination of patients with diabetes by health professionals was higher in the Southeast and Minor regions of the Central-West Region. Non-predominance of the test was more prevalent among females, in rural, color/race, single, uninstructed or incomplete elementary school subjects. In Brazil, only 58.4% of diabetic patients receiving regular consultation received guidance to examine the feet. The occurrence of sores was important for males; in black people/pardo Brazilians. The presence of foot wound was more significant among diabetic patients who had a regular visit. Conclusion Thus, in Brazil, one of the guidelines for the prevention of diabetic ulcers is precarious, with lower prevalence of care in the most vulnerable groups.

Objetivo Analizar las tendencias en la incidencia y mortalidad por cancer de pancreas en los paises latinoamericanos. Metodo Se realizo un estudio ecologico con datos de incidencia de la Agencia Internacional de Investigacion sobre Cancer y datos de mortalidad de la Organizacion Mundial de la Salud. Se analizo la tendencia de incidencia por regresion de Joinpoint, la variacion del promedio anual y el intervalo de confianza del 95%. Resultados Hubo tendencias crecientes en la incidencia en Brasil en varones entre 40 y 59 anos, y una reduccion en Costa Rica. En las mujeres, hubo estabilidad en todos los grupos de edad. Las tasas de mortalidad aumentaron en los ancianos en Brasil (AAPC: 1,09%; IC 95%: 0,76; 1.42), Peru (AAPC: 1,76%; IC 95%: 0,36; 3,17) y El Salvador (AAPC: 2,88%; 95% IC 0,38; 5,43), mientras que en Mexico hubo una reduccion. En las mujeres, esta tasa aumento en Brasil (AAPC: 1,38%; IC 95%: 1,07; 1.69), Peru (AAPC: 2,25%; IC 95%: 0,68; 3.85), Chile (AAPC: 3,62%; IC 95%: 1,96; 5,31), Nicaragua (AAPC: 2,51%; IC 95%: 0,36; 4,71) y Paraguay (AAPC: 1,17%; IC 95%: 0,37; 1,98) y se observo una tendencia a la baja en Colombia y Ecuador. Conclusiones El cancer de pancreas tuvo una mayor incidencia en la poblacion anciana de ambos sexos; cabe tambien senalar que debido a este aumento su mortalidad en las mujeres.
