Computerised interpretation of fetal heart rate during labour (INFANT): a randomised controlled trial
Continuous electronic fetal heart-rate monitoring is widely used during labour, and computerised interpretation could increase its usefulness. We aimed to establish whether the addition of decision-support software to assist in the interpretation of cardiotocographs affected the number of poor neonatal outcomes. In this unmasked randomised controlled trial, we recruited women in labour aged 16 years or older having continuous electronic fetal monitoring, with a singleton or twin pregnancy, and at 35 weeks' gestation or more at 24 maternity units in the UK and Ireland. They were randomly assigned (1:1) to decision support with the INFANT system or no decision support via a computer-generated stratified block randomisation schedule. The primary outcomes were poor neonatal outcome (intrapartum stillbirth or early neonatal death excluding lethal congenital anomalies, or neonatal encephalopathy, admission to the neonatal unit within 24 h for ≥48 h with evidence of feeding difficulties, respiratory illness, or encephalopathy with evidence of compromise at birth), and developmental assessment at age 2 years in a subset of surviving children. Analyses were done by intention to treat. This trial is completed and is registered with the ISRCTN Registry, number 98680152. Between Jan 6, 2010, and Aug 31, 2013, 47 062 women were randomly assigned (23 515 in the decision-support group and 23 547 in the no-decision-support group) and 46 042 were analysed (22 987 in the decision-support group and 23 055 in the no-decision-support group). We noted no difference in the incidence of poor neonatal outcome between the groups—172 (0·7%) babies in the decision-support group compared with 171 (0·7%) babies in the no-decision-support group (adjusted risk ratio 1·01, 95% CI 0·82–1·25). At 2 years, no significant differences were noted in terms of developmental assessment. Use of computerised interpretation of cardiotocographs in women who have continuous electronic fetal monitoring in labour does not improve clinical outcomes for mothers or babies. McCain's attempt to woo woman voters, especially disgruntled [Hillary] supporters, may ultimately backfire, as thoughtful woman will vote for the ticket that they perceive as best for their families and the country, not gender. So far [Sarah Palin] offers no expertise on important national and international issues; and she appears to stand on the side of overturning Roe v. Wade, which most women favor. (Women gun enthusiasts will love her, however.) Moreover, questions are already buzzing in the media about Palin's alleged ethics and hypocrisy. Even though the good folks attending the RNC have circled the wagons around her, if most voters -- men and women -- perceive Palin as a weak candidate with extreme views and character issues, then McCain's judgment in his selection of a running mate -- someone he met only briefly before selecting her -- might do him more harm than good. Voters will be able to better judge Palin's credentials after she is allowed to give press conferences with the national media and be interviewed on \"Meet The Press.\"
Silver Hawks need to build fan loyalty
Of course, the Silver Hawks club can't sustain itself, and it won't remain in South Bend, unless the community supports it. But it's not enough to expect folks to show up simply because they should. It's up to the Silver Hawks organization to deliver a valued product on the field and an affordable entertainment package that will build a loyal fan base.

Structure and mechanism of cooperation by Escherichia coli hsp40 DnaJ and hsp70 DnaK
The Escherichia coli hsp40 DnaJ and hsp70 DnaK cooperate in the binding of proteins at intermediate stages of folding, assembly and translocation across membranes. Binding of protein substrates to the DnaK C-terminal domain is controlled by ATP binding and hydrolysis in the N-terminal ATPase domain. The interaction of DnaJ with DnaK is mediated at least in part by the highly conserved N-terminal J-domain of DnaJ that includes residues 2 to 75. The goal of this research is to understand at the atomic level how hsp40 and hsp70 cooperate as molecular chaperones in the management of protein-protein interactions. This dissertation consists of two parts: (I) The interaction of the J-domain, DnaJ2-75, with full-length DnaK and with the ATPase domain of DnaK, DnaK2-388, is characterized; (II) the folding and stability of 5-hydroxytryptophan-containing DnaJ variant proteins are described. In part I, residues in the J-domain involved in the interaction with DnaK were identified by heteronuclear NMR experiments. The J-domain interaction is localized to the ATPase domain of DnaK and is likely to be dominated by electrostatic interactions. Dissociation constants were calculated for the interaction of DnaJ2-75 for DnaK(MgADP) and DnaK2-388(MgADP). NMR and paramagnetic relaxation experiments examined the proximity of DnaJ2-75 to Mn2+ in DnaK-bound MnADP, and the results suggest that DnaJ2-75 does not approach within five angstroms of Mn2+ in a DnaK(MnADP) complex. In part II, DnaJ variant proteins containing 5-hydroxytryptophan (5HW) were characterized by unfolding and fluorescence, circular dichroism and transverse urea gradient gel electrophoresis. Unfolding monitored by fluorescence of 5HW residues located in the J-domain is cooperative, whereas unfolding monitored by a 5HW residue in the gly/phe-rich region is not cooperative. Unfolding of one of the DnaJ 5HW variants is cooperative and at least partially reversible. Fluorescence quenching with iodide of 5HW in variant proteins suggests that the gly/phe-rich region is more accessible to solvent than the J-domain. This is consistent with the proposal that the gly/phe-rich region is unstructured. Structural data obtained in these studies contribute toward a detailed mechanism of hsp40/hsp70 cooperation. DnaJ can interact with certain polypeptide substrates prior to DnaK. Therefore, we propose that DnaJ enhances the ability of DnaK to bind polypeptide substrates by targeting DnaK to the complex, stimulating ATP hydrolysis of DnaK and thereby stabilizing the ternary DnaJ-DnaK(MgADP)-polypeptide complex.
Pentagon cutbacks could lead to more jobs at Nellis, test site
The latest in a series of proposed Pentagon cutbacks could mean a few more jobs and greater military consolidation at Nellis Air Force Base and the Nevada Test Site. A report released Tuesday by the Department of Defense recommended that two units from Air Force bases scheduled for realignment be transferred to Nellis and its 3-million-acre range, bringing an estimated 87 military positions and 85 civilian jobs to Nevada. The Defense Nuclear Agency's Field Command, currently at Kirtland Air Force Base in New Mexico, would be the most significant of the proposed moves. That agency tracks and manages the nation's nuclear weapons stockpile, conducts research on the effects of nuclear weapons, and studies ways to thwart their proliferation.
Lincoln County not warned about hantavirus case
State health officials confirmed Monday that they failed to warn Lincoln County authorities that they suspected a Las Vegas man may have contracted the deadly hantavirus a month ago while working at the Nellis Air Force Range complex. \"By policy, we should have, once we knew that it was hantavirus, and once we knew that this person was in a setting where he could have contracted it in Lincoln County, Lincoln County health officials should have been notified,\" Craigie said. Lonnie Empey, a Clark County environmental health supervisor, said Monday authorities planned to wait a month for blood test results before warning REECo, Air Force and Lincoln County authorities of possible hantavirus exposure.
