PACU discharge: equally safe and more cost-effective than floor admission for uncomplicated laparoscopic appendectomy

Background
Laparoscopic appendectomy is a common emergency surgical procedure worldwide, known for its benefits of reduced pain, shorter hospital stays, and quicker recovery times. Although postoperative care typically involves observation on the surgical floor, advances in surgical techniques and perioperative care have introduced the potential for discharging patients directly from the post-anesthesia care unit (PACU). This study aims to evaluate the safety and cost-effectiveness of direct PACU discharge compared to traditional floor admission for patients undergoing uncomplicated laparoscopic appendectomy.
Methods
This retrospective cohort study analyzed adult patients diagnosed with uncomplicated appendicitis between January 2021 and December 2023. Patients were divided into two cohorts: those discharged directly from PACU and those admitted to the floor before discharge. Primary outcomes included 30-day readmission rates, reoperation rates, and postoperative complications. Secondary outcomes assessed costs, surgery times, and demographic variables. Results
A total of 203 patients were included, with 103 in the PACU cohort and 100 in the floor cohort. PACU patients were younger and had fewer comorbidities than floor patients. No significant differences were found in 30-day readmission, reoperation rates, or complications between the groups. PACU discharge was associated with significantly shorter hospital stays (8 h vs. 26 h,

< 0.001) and lower costs, with average charges of $27,739 for PACU discharges versus $31,593 for floor discharges, primarily due to reduced labor costs.
Conclusion
Direct discharge from the PACU following uncomplicated laparoscopic appendectomy is both safe and cost-effective compared to floor admission. These findings suggest that PACU discharge is a viable option for well-selected patients, with the potential for significant healthcare savings. Future research should focus on refining patient selection criteria and validating these findings in diverse healthcare settings.

Baseball Related Injuries: A Case Report on Acute Compartment Syndrome of the Forearm

Acute compartment syndrome is a difficult diagnosis to make due to its wide range of clinical presentations. Delay or misdiagnosis can cause devastating consequences such as Volkmann’s ischemic contracture, permanent nerve damage, amputation, and death. Lower extremity compartment syndrome is more common than upper extremity compartment syndrome, with the forearm being the most common location for upper extremity compartment syndrome. Acute compartment syndrome is most caused by acute fracture trauma but can also be due to soft tissue crush injuries or vascular problems. We report a unique case of a male umpire being struck on the forearm by a baseball with subsequent progression to an acute compartment syndrome that required emergent fasciotomies. The patient made a full recovery with no known long-term sequelae.

Unmanned Aerial Systems: Shaping Asymmetric Conflicts and Implications for U.S. Dominance
Unmanned Aerial Systems (UAS) have reshaped military strategies and capabilities, creating opportunities and challenges for combatants. Research within the available literature revealed a gap in tactical methods for UAS deployment. The researcher examined UAS usage in Yemen, Gaza, and Ukraine from January 1, 2021, to January 1, 2024, focusing on how employment methods affect battlefield outcomes and U.S. dominance. The researcher addressed the questions: How can the Joint Force Commanders best align UAS capabilities to support joint operational requirements, how can the U.S. advance UAS technologies and tactics to optimize contributions and sustain its military advantage, and what are the underlying causes that have hindered the successful integration of manned and unmanned aerospace systems for effectively countering challenges from peer and near-peer adversaries?The sample in this research is the conflicts in Yemen, Gaza, and Ukraine. Employing a qualitative research design and a multiple-case study approach, the researcher examined the employment methods of UAS in Yemen, Gaza, and Ukraine. Data analysis revealed that combatants in Yemen and Ukraine favor one-way UAS, while in Gaza, combatants prefer reusable UAS. The results revealed that the U.S. military must enhance UAS capabilities and integrate UAS into command and control (C2) frameworks. The conclusion is that UAS procurement and advancing AI capabilities are essential for U.S. dominance. The implications of this research are that Joint Force Commanders should integrate UAS into C2 structures leveraging their expendable nature to enhance operational flexibility. Future research should focus on optimizing UAS capabilities and developing UAS countermeasures.

Trauma remains a significantly underfunded area of medical research despite its status as a leading cause of death and substantial economic burden in the United States. This study explores the disparity in trauma research funding, focusing on Oklahoma compared to neighboring and populous states.
Using data from the Centers for Disease Control and Prevention (CDC)'s Web-based Injury Statistics Query and Reporting System (WISQARS™) and the National Institutes of Health (NIH)'s RePORT databases, we analyzed age-adjusted years of potential life lost (YPLL) due to trauma and corresponding NIH funding across different states and nationally. Statistical analyses included bivariate comparisons and standardization of funding data per population and YPLL.
From 2010 to 2020, NIH allocated approximately $124 billion nationally, with only 5% dedicated to trauma, amounting to $1,772.32 per age-adjusted YPLL. Oklahoma and Kansas exhibited the lowest NIH funding per YPLL compared to New York and California. Funding for the National Injury Prevention and Control, a subdivision of the CDC, has shown significant increases, ranking third in 2022.
This study highlights the urgent need for enhanced trauma research funding to address its disproportionate impact on mortality and healthcare costs. Strategic allocation of resources is essential to advance trauma care and align research priorities with public health needs.

Injury Risk in Pole Vaulters: The Impact of Landing Technique and Experience Level in an Observational Study of 1,012 Athletes and 6,751 Attempts

Pole vaulting is a complex track and field event where athletes sprint down a runway, using a flexible pole to propel themselves over a horizontal crossbar. While safety regulations have reduced catastrophic injury rates, lower extremity injuries remain prevalent and demand further investigation. However, injury risks remain a concern due to the complexity of the sport and repetitive fall from heights. This study aims to investigate the correlation between landing mechanics and injury rates in pole vaulting. The authors hypothesize that improper landing techniques significantly increase the risk of injury, particularly in less experienced athletes.
This study was conducted at a national indoor pole vault event consisting of 1,012 athletes, ranging from beginner to elite level. Data was collected on skill level, landing location and position, and injuries. Only injuries sustained during a competitive vault attempt were considered for data analysis, which included descriptive statistics.
The landing injury rate was low at 0.12%, with eight injuries occurring in 6,751 vault attempts. Notably, no injuries occurred in athletes with optimal landing mechanics. All reported injuries affected the lower extremities and were exclusively observed in less experienced vaulters.
Though pole vaulting is a relatively safe sport, catastrophic injuries are possible and the injuries that do occur are preventable, as this study reveals they occur at a higher rate in inexperienced vaulters as well as with an improper landing technique. Coaches and athletes should emphasize proper landing techniques to prevent injuries and improve performance.

Feasibility of Passive Electromagnetic Dampers as Energy Harvesters in Large Structures
2013
There has been a trend in structural design toward energy efficient design and motion based design. The strategy of motion based design is controlling the movement of structures to meet certain dynamic response requirements by damping the structure. Structural damping dissipates the energy of external loads internally within the structure. A simple idea is to connect the two design strategies to control the motion of a structure while harvesting this dissipated energy by transducing it to electrical energy via passive electromagnetic damping. This study will attempt to determine the feasibility of using passive electromagnetic damping to control the motion and harvest the energy of damping of large scale structures.

Porcine circoviruses (PCVs), including porcine circovirus 2 (PCV2) and porcine circovirus 3 (PCV3), have been associated with clinical syndromes in swine, resulting in significant economic losses. To better understand the epidemiology and clinical relevance of PCV2 and PCV3, this study analyzed a dataset comprising diagnostic data from six veterinary diagnostic laboratories (VDLs) in the United States of America. The data comprised of polymerase chain reaction (PCR) test results, sample type, and age group for PCV2 and PCV3 submissions from 2002–2023. Findings indicated a decrease in the percentage of PCV2-positive submissions after introducing a commercial PCV2 vaccine in 2006 and a resurgence in positivity after 2018, particularly in breeding herds, associated with an increased number of submissions using processing fluid samples. After its first report in the U.S. in 2016, PCV3 detection had an upward trend in the percentage of positive cases, peaking in spring 2023. PCV3 detection was more frequent in adult/sow farms, while PCV2 was more frequently detected in the wean-to-market category. An additional analysis used results from tissue diagnostic data from 2019–2023 from one VDL to associate PCR cycle threshold (Ct) values with the probability of confirming a PCV2 or PCV3 disease diagnosis confirmation. An interpretative PCR Ct cutoff for PCV2 and PCV3 diagnoses was assessed based on the logistic regression model associating Ct values with the presence of tissue lesions. The analysis considered only cases tested for PCV2 and PCV3 by PCR with tissue evaluations by diagnosticians. An interpretative Ct cutoff of 22.4 for PCV2 was associated with a high probability of confirming a diagnosis of PCV2 clinical disease through histopathology. For PCV3, the interpretative cutoff with the highest performance was 26.7. These findings contribute to the ongoing efforts to monitor and understand the clinical relevance of PCV2 and PCV3 PCR results, identifying potential disease challenges.

Vasculogenic mimicry in small cell lung cancer

Small cell lung cancer (SCLC) is characterized by prevalent circulating tumour cells (CTCs), early metastasis and poor prognosis. We show that SCLC patients (37/38) have rare CTC subpopulations co-expressing vascular endothelial-cadherin (VE-cadherin) and cytokeratins consistent with vasculogenic mimicry (VM), a process whereby tumour cells form ‘endothelial-like’ vessels. Single-cell genomic analysis reveals characteristic SCLC genomic changes in both VE-cadherin-positive and -negative CTCs. Higher levels of VM are associated with worse overall survival in 41 limited-stage patients’ biopsies (
P
<0.025). VM vessels are also observed in 9/10 CTC patient-derived explants (CDX), where molecular analysis of fractionated VE-cadherin-positive cells uncovered copy-number alterations and mutated
TP53,
confirming human tumour origin. VE-cadherin is required for VM in NCI-H446 SCLC xenografts, where VM decreases tumour latency and, despite increased cisplatin intra-tumour delivery, decreases cisplatin efficacy. The functional significance of VM in SCLC suggests VM regulation may provide new targets for therapeutic intervention.
