Zoonotic diseases: understanding the risks and mitigating the threats
Zoonotic diseases are like a sneaky game of “tag” between animals and humans, where the stakes are high and the consequences can be deadly. From the bubonic plague to COVID-19, zoonotic diseases have affected humanity for centuries, reminding us of our interconnectedness with the animal kingdom and the importance of taking proactive measures to prevent their spread. Whether it is avoiding contact with animals or practicing good hygiene, staying safe from zoonotic diseases is a game we all need to play.
Which flexible ureteroscope is the best for upper tract urothelial carcinoma treatment?
PurposeTo present attributes of currently available flexible ureteroscopes to define the best flexible ureteroscope for upper tract urothelial carcinoma (UTUC) treatment.Materials and methodsScopus and Medline databases were searched for articles relating to performance of flexible ureteroscopes. A consensus for final inclusion of articles judged to be relevant for UTUC treatment was reached between the authors. Instrument characteristics were extracted from manufacturers’ product brochures.ResultsSmaller cross-sectional size of instruments is associated with increased probability for successful primary access to the upper urinary tract. The smallest flexible ureteroscopes are fiberoptic scopes. Smaller ureteroscopes also allow comparatively increased irrigation flow at constant intrarenal pressure. Digital flexible ureteroscopes achieve superior image quality compared to their fiberoptic counterparts, at the price of lower end-deflection ability. Image enhancement technologies such as narrow-band imaging (NBI), photodynamic diagnosis (PDD) and Image 1-S (formerly SPIES) are based on subjective image interpretation by the operator. NBI and PDD significantly increase tumor detection rate. The highest subjective image quality score of the Image 1-S technology is reached by the “Clara + Chroma” mode. Single-use ureteroscopes offer potential advantages over reusable scopes, including sterility, absence of contamination, immediate availability and exemption of previous instrument wear.ConclusionsMiniaturization, digital image caption and image enhancement technologies seem to be the major determinants defining the best flexible ureteroscope for UTUC treatment. The impact of further factors, such as distal tip design, torque, working channel position, risk of contamination, as well as upcoming technological innovations should be evaluated in randomized controlled trials.
Predicting depressed and elevated mood symptomatology in bipolar disorder using brain functional connectomes
The study is aimed to identify brain functional connectomes predictive of depressed and elevated mood symptomatology in individuals with bipolar disorder (BD) using the machine learning approach Connectome-based Predictive Modeling (CPM). Functional magnetic resonance imaging data were obtained from 81 adults with BD while they performed an emotion processing task. CPM with 5000 permutations of leave-one-out cross-validation was applied to identify functional connectomes predictive of depressed and elevated mood symptom scores on the Hamilton Depression and Young Mania rating scales. The predictive ability of the identified connectomes was tested in an independent sample of 43 adults with BD. CPM predicted the severity of depressed [concordance between actual and predicted values ( = 0.23, = 0.031) and elevated ( = 0.27, = 0.01) mood. Functional connectivity of left dorsolateral prefrontal cortex and supplementary motor area nodes, with inter- and intra-hemispheric connections to other anterior and posterior cortical, limbic, motor, and cerebellar regions, predicted depressed mood severity. Connectivity of left fusiform and right visual association area nodes with inter- and intra-hemispheric connections to the motor, insular, limbic, and posterior cortices predicted elevated mood severity. These networks were predictive of mood symptomatology in the independent sample ( ⩾ 0.45, = 0.002). This study identified distributed functional connectomes predictive of depressed and elevated mood severity in BD. Connectomes subserving emotional, cognitive, and psychomotor control predicted depressed mood severity, while those subserving emotional and social perceptual functions predicted elevated mood severity. Identification of these connectome networks may help inform the development of targeted treatments for mood symptoms.
Effects of ebselen addition on emotional processing and brain neurochemistry in depressed patients unresponsive to antidepressant medication
Lithium is an effective augmenting agent for depressed patients with inadequate response to standard antidepressant therapy, but numerous adverse effects limit its use. We previously reported that a lithium-mimetic agent, ebselen, promoted a positive emotional bias—an indicator of potential antidepressant activity in healthy participants. We therefore aimed to investigate the effects of short-term ebselen treatment on emotional processing and brain neurochemistry in depressed patients with inadequate response to standard antidepressants. We conducted a double-blind, placebo-controlled 7-day experimental medicine study in 51 patients with major depressive disorder who were currently taking antidepressants but had an inadequate response to treatment. Participants received either ebselen 600 mg twice daily for seven days or identical matching placebo. An emotional testing battery, magnetic resonance spectroscopy and depression and anxiety rating scales were conducted at baseline and after seven days of treatment. Ebselen did not increase the recognition of positive facial expressions in the depressed patient group. However, ebselen increased the response bias towards fear emotion in the signal detection measurement. In the anterior cingulate cortex, ebselen significantly reduced the concentrations of inositol and Glx (glutamate+glutamine). We found no significant differences in depression and anxiety rating scales between visits. Our study did not find any positive shift in emotional bias in depressed patients with an inadequate response to antidepressant medication. We confirmed the ability of ebselen to lower inositol and Glx in the anterior cingulate cortex. These latter effects are probably mediated through inhibition of inositol monophosphatase and glutaminase respectively.
Graph theory analysis of whole brain functional connectivity to assess disturbances associated with suicide attempts in bipolar disorder
Brain targets to lower the high risk of suicide in Bipolar Disorder (BD) are needed. Neuroimaging studies employing analyses dependent on regional assumptions could miss hubs of dysfunction critical to the pathophysiology of suicide behaviors and their prevention. This study applied intrinsic connectivity distribution (ICD), a whole brain graph‐theoretical approach, to identify hubs of functional connectivity (FC) disturbances associated with suicide attempts in BD. ICD, from functional magnetic resonance imaging data acquired while performing a task involving implicit emotion regulation processes important in BD and suicide behaviors, was compared across 40 adults with BD with prior suicide attempts (SAs), 49 with BD with no prior attempts (NSAs) and 51 healthy volunteers (HVs). Areas of significant group differences were used as seeds to identify regional FC differences and explore associations with suicide risk-related measures. ICD was significantly lower in SAs than in NSAs and HVs in bilateral ventromedial prefrontal cortex (vmPFC) and right anterior insula (RaIns). Seed connectivity revealed altered FC from vmPFC to bilateral anteromedial orbitofrontal cortex, left ventrolateral PFC (vlPFC) and cerebellum, and from RaIns to right vlPFC and temporopolar cortices. VmPFC and RaIns ICD were negatively associated with suicidal ideation severity, and vmPFC ICD with hopelessness and attempt lethality severity. The findings suggest that SAs with BD have vmPFC and RaIns hubs of dysfunction associated with altered FC to other ventral frontal, temporopolar and cerebellar cortices, and with suicidal ideation, hopelessness, and attempt lethality. These hubs may be targets for novel therapeutics to reduce suicide risk in BD.
A Constructionist and Corpus-Based Approach to Formulas in Old English Poetry
This paper explores a constructionist and corpus-based approach to Old English formulaic language through an analysis of the “maþelode system” of speech introductions. The analysis is performed on a section of the York-Helsinki Parsed Corpus of Old English Poetry, comprising the poems Beowulf, Battle of Brunanburh, and Exodus. The results show that most instances of the maþelode system belong to a well-attested construction continuum, structured by the widespread Old English (and ultimately Germanic) poetic devices of variation and kenning. This continuum ranges from more fixed repetitions that exclusively involve the verb maþelian to more schematic patterns that are also attested by other speech verbs, by verbs of giving, as well as by a number of further verbs of various semantic types. The particularly high frequency of this pattern with speech verbs and verbs of giving matches the prominent role, highlighted by previous studies, of both word-exchange and gift-exchange within Old English heroic ideology, and suggests that these formulaic patterns served the purpose to characterize the protagonists of speech or giving events as heroic and/or lordly figures.
Spatial Analysis of Infections by Toxoplasma gondii and Neospora caninum (Protozoa: Apicomplexa) in Small Ruminants in Northern Italy
The objectives of this study were: (i) To investigate possible geographical or environmental factors influencing the infections by Toxoplasma gondii and Neospora caninum in sheep and goats in northern Italy; (ii) to identify areas at risk of infection to set up preventive measures. Forty-three sheep and goat farms were included. Their locations were plotted and associated with T. gondii and N. caninum seroprevalence, then the distribution of farms’ prevalence was evaluated by spatial analysis. Significant clusters for both low and high prevalence were obtained, and a generalized linear model with ordinal logistic regression was implemented to verify if spatial clustering could be due to climate factors (temperature, rainfall, and their interaction). Clusters of high (80.0%) and low prevalence (28.12%) resulted for T. gondii seroprevalence in sheep farms. No significant clusters resulted for goat farms. Clusters of high (38.68%) and low prevalence (21.23%) resulted for N. caninum seroprevalence in sheep farms. One high-prevalence cluster (15.62%) resulted for goat farms. For goats, spatial analysis and analysis on climatic data showed the absence of environmental significant risk factors associated with T. gondii or N. caninum infection. On the contrary, for sheep, annual temperature, rainfall, and their association affected the risk of T. gondii and N. caninum infection. Particularly, high temperatures and abundant rainfalls were related to T. gondii seroprevalence, while low temperatures and scarce rainfalls were related to N. caninum seroprevalence.
First report of Besnoitia bennetti in Irish donkeys: an emerging parasitic disease in Europe
Background This is the first report of Besnoitia bennetti in donkeys in Ireland. B. bennetti , an apicomplexan protozoan parasite specific to equids, is an emerging pathogen in Europe. This parasite forms chronic intracytoplasmic cysts in cells of the mesenchymal lineage, mainly fibroblasts, in the skin, sclera and mucosa. Clinical signs in affected equine hosts vary from mild to severe debilitating disease. Little is known of the phylogeny, epidemiology or transmission of B. bennetti infection in donkeys, mules or horses. Case presentation Two cases of besnoitiosis in donkeys are presented. Both donkeys were born and raised in theSouthwest of Ireland. The first case was a 2.5-year-old donkey that had a suspect sarcoid removed, while the second case,a 2-year-old donkey, had a biopsy of nodular dermatitis of the muzzle. Diagnosis was made by histopathology and the parasite species, B. bennetti , was confirmed by PCR followed by sequencing and microsatellite analysis. Both donkeys had high antibody titres against Besnoitia spp. Small (0.5 mm) scleral, conjunctival and dermal cysts over the muzzle were subsequently observed in both animals. Treatment with trimethoprim sulfadiazine for 30 days did not lead to clinical resolution. The findings are compared to the cases of besnoitiosis in donkeys reported in the past 10 years throughout Europe. Conclusions Besnoitiosis should be considered as a differential diagnosis for chronic skin disease particularly in cases of cutaneous masses, non-pruritic dermatitis, and dermatitis that is not responsive to treatment in donkeys and other equids. Future studies are needed to investigate the prevalence of the disease in Irish donkeys, the spread of the disease and the potential impact on the health and welfare of the donkeys.
Serological Prevalence of Toxoplasma gondii, Neospora caninum, and Sarcoptes scabiei var. suis in Wild Boars (Sus scrofa) Hunted in a Highly Anthropized Area in Italy
Due to the increasing expansion into urban and rural areas, wild boars represent a potential source of infection with zoonotic and animal-specific parasites for both humans and animals. Therefore, the aim of this study was to investigate the serological prevalence to Toxoplasma gondii, Neospora caninum, and Sarcoptes scabiei var. suis in blood samples from wild boars (Sus scrofa) hunted in an anthropized area in Italy. Enzyme-linked immunosorbent assay (ELISA) tests were used to detect antibodies anti-T. gondii and anti-S. scabiei and an immunofluorescence antibody test (IFAT) for antibodies anti-N. caninum. 81 out of 128 wild boars (P = 63.3%) resulted positive for at least one of the three parasites. 68 of them were seropositive to T. gondii (P = 53.1%) and 14 to N. caninum (P = 10.9%). 9 wild boars resulted seropositive to S. scabiei var. suis (P = 7.0%). Sampling season was the only significant risk factor related to S. scabiei var. suis seroprevalence (OR = 7.8). The high occurrence of T. gondii supports the role of this species as a source of infection for other animals and humans. Furthermore, the serological prevalence of N. caninum and S. scabiei var. suis in wild boars from the study area characterized by the presence of numerous dairy cattle and pig farms is relevant to demonstrate its suitability for the circulation of these parasites both in domestic and wild species.
The Utility of Serological Analysis for Neospora caninum Infection in Dairy Cattle Farms Management: Serological Investigation and Evaluation of the Effects on Reproductive and Productive Performances in Two Study Herds in Northern Italy
Neospora caninum is recognized as a major cause of abortion in cattle, determining economic losses, particularly in dairy industries. To evaluate the impact of neosporosis on herd efficiency, and to understand how the serological analysis for N. caninum is explicative of the farm picture, an epidemiological study was designed in two dairy farms recruited as a case study. Blood samples were collected from 540 animals, including cows and heifers over 12 months, and analyzed by an indirect immunofluorescent antibody test with subsequent antibody titration. Overall, 94 animals (17.4%) were identified as positive to N. caninum antibodies (15.5% and 18.5% in Farm 1 and Farm 2), with differences between the farms concerning the antibody titers (Chi-square, p-value = 0.04), particularly in cows (Chi-square, p-value = 0.018). Consequently, a different pattern of abortion episodes was depicted in the two investigated farms. Considering reproductive and productive performances, the number of inseminations necessary to make an animal pregnant was higher in seropositive cows (2.4 and 2.9 in Farm 1 and 2, respectively) than in seronegative ones (2.1 and 2.4 in Farm 1 and 2, respectively). Similarly, particularly in Farm 1, the number of days in milk of not-pregnant cows was higher in seropositive (167.7) than seronegative animals (133.4). Moreover, although the association between N. caninum infection and milk production is still unclear, both the daily milk production and the 305-mature equivalent milk yield were lower in seropositive (31.02 and 11,838.94) than seronegative cows (33.59 and 12,274.88) in Farm 1; an opposite pattern was otherwise depicted in Farm 2. The study showed that even if N. caninum circulated equally in the two herds, the dynamics of the parasite infection and its outcome may be different, also depending on the specific situation of the farm. In this way, the integration of serological analysis for N. caninum, the reproductive and productive parameters, and the information on herd performances, could give specific indications for the application of control strategies.
