Background
FTO is known to be associated with body mass and obesity in humans and its over-expression affects the energy metabolism of cancer cells. The aim of the present study is to investigate the biological role of FTO in human bladder urothelial carcinoma.
Methods
PCR and western blotting are used to measure the levels of FTO in both tissues and cell lines (5637, T24, TCCSUP) of human bladder urothelial carcinoma. Raw RNA-Sequencing reads and the corresponding clinical information for bladder urothelial carcinoma are downloaded from TCGA. Cell Counting Kit-8 and wound healing assays are used to explore the effect of FTO on proliferation and migration of bladder cancer cells.
Results
The expression of FTO mRNA in bladder urothelial carcinoma decreases significantly compared with the normal controls from both the data of real-time PCR (
p
< 0.05) and TCGA (
p
< 0.01). Loss-of-function assays revealed that knockdown of FTO significantly promotes proliferation and migration of 5637 and T24 cells. Consistently, we found that the cisplatin-induced cytotoxicity of bladder cancer cell could be rescued by co-treatment with MA2, which was previously reported as a highly selective inhibitor of FTO, compared with the cisplatin-control group.
Conclusions
These findings suggest that down-regulation of FTO plays an oncogenic role in bladder cancer. The further exploration of regulation of FTO expression may provide us a potential therapeutic target for the treatment of bladder cancer.

Expression signature of six‐snoRNA serves as novel non‐invasive biomarker for diagnosis and prognosis prediction of renal clear cell carcinoma

Increasing evidence has verified that small nucleolar RNAs (snoRNAs) play significant roles in tumorigenesis and exhibit prognostic value in clinical practice. In the study, we analysed the expression profile and clinical relevance of snoRNAs from TCGA database including 530 ccRCC (clear cell renal cell carcinoma) and 72 control cases. By using univariate and multivariate Cox analysis, we established a six‐snoRNA signature and divided patients into high‐risk or low‐risk groups. We found patients in high‐risk group had significantly shorter overall survival and recurrence‐free survival than those in low‐risk group in test series, validation series and entire series by Kaplan‐Meier analysis. We also confirmed this signature had a great accuracy and specificity in 64 clinical tissue cases and 50 serum samples. Then, depending on receiver operating characteristic curve analysis we found the six‐snoRNA signature was an superior indicator better than conventional clinical factors (AUC = 0.732). Furthermore, combining the signature with TNM stage or Fuhrman grade were the optimal indicators (AUC = 0.792; AUC = 0.800) and processed the clinical applied value for ccRCC. Finally, we found the SNORA70B and its hose gene USP34 might directly regulate Wnt signalling pathway to promote tumorigenesis in ccRCC. In general, our study established a six‐snoRNA signature as an independent and superior diagnosis and prognosis indicator for ccRCC.

The anesthesiologist’s perspective in choosing between robotic-assisted and conventional laparoscopic urological surgery: a propensity score-matched analysis

Background
Comparative studies between robotic-assisted laparoscopic surgery (RALS) and conventional laparoscopic surgery (CLS) primarily focus on surgical outcomes, with limited emphasis on the anesthesiologist's perspective. This study aimed to compare anesthesia-related Enhanced Recovery After Surgery (ERAS) outcomes to inform surgical decision-making.
Methods
This retrospective study included 831 patients who underwent partial nephrectomy or radical prostatectomy for renal or prostate cancer between September 2021 and October 2023. Propensity score matching (PSM) was performed in a 1:1 ratio based on age, gender, BMI, surgery type, and comorbidities. Primary outcomes included anesthesia time, surgical time, recovery time, and resting/activity VAS scores on postoperative day one (POD1). Secondary outcomes comprised ambulation status, nausea/vomiting, dizziness, and anxiety incidence. Safety indicators were pharyngeal pain, respiratory depression, and oxygen requirement.
Results
After PSM, 252 patients were allocated to each group. The RALS group demonstrated significantly longer anesthesia and surgical times compared to the CLS group (
P
< 0.05). PACU recovery time showed no significant difference. While Rest-VAS on POD1 was similar, Act-VAS was significantly higher in the RALS group (
P
<0.05). The proportion of patients achieving autonomous mobilization (Grade 1) was significantly lower in the RALS group(
P
<0.01). Incidences of nausea/vomiting and dizziness were significantly higher in the RALS group (
P
<0.05). Anxiety incidence was significantly lower in the RALS group (
P
<0.05). No significant differences were found in sore throat, respiratory depression, or oxygen requirement rates.
Conclusions
These findings suggest that from an anesthesiological perspective, RALS is associated with longer surgical and anesthesia time. The observed differences in postoperative recovery indicators highlight the importance of considering patient physiological reserve when selecting a surgical approach. However, given the observational nature of this study, these findings should be considered hypothesis-generating and warrant prospective validation.

LLM-BCgrading: Large language model-based Chinese medical long text classification for bladder cancer grade prediction

Background
Traditional cystoscopic biopsy-based methods for histological grading of bladder cancer (BC) are invasive, subject to sampling errors, and susceptible to interobserver variability among pathologists. To address these challenges, this study explores a large language model (LLM)-based noninvasive approach to BC grade prediction using long Chinese medical texts.
Methods
We retrospectively collected admission records and computed tomography urography (CTU) descriptions from 642 patients pathologically diagnosed with BC. Each paired text was annotated as low grade or high grade according to histopathological results. We developed LLM-BCgrading to leverage HuatuoGPT-7B for Chinese medical long-text representation and integrated a gated multiplicative attention mechanism (GMAM) to selectively emphasize discriminative features. To address class imbalance and clinical risk asymmetry, the model was optimized with a cost-sensitive loss function. Performance was evaluated on a fixed internal test set with additional evaluation on an independent external validation cohort to assess generalizability.
Results
The best-performing configuration combined both admission records and CTU descriptions via an attention-based fusion strategy and GMAM, achieving balanced accuracy of 0.757, macro F1 score of 0.749, and macro AUC of 0.740. The ablation results demonstrated that incorporating both texts significantly improved classification performance compared with single-text configurations, and the GMAM consistently outperformed conventional attention mechanisms. Dimensionality experiments identified 256 as the optimal embedding size, balancing computational efficiency and predictive performance.
Conclusion
Our findings demonstrate that LLMs can effectively process Chinese medical long-texts for accurate preoperative prediction of BC grade. Attention-based fusion, cost-sensitive optimization, and interpretability based on Shapley additive explanations further support the robustness and clinical relevance of this LLM-driven framework.

China is rich in medicinal–ornamental plants with multifunctional uses, making a significant contribution to global landscaping, environmental beautification, and the health industry. In the post-pandemic era, there is an increasing focus on improving living environments and enhancing immune health, leading to a growing demand for the development and utilization of these plant resources. Resource evaluation is fundamental to their widespread application in landscaping, commercial production, germplasm innovation, and sustainable utilization. However, current research is limited, and there is an absence of a comprehensive evaluation system. The evaluation of these plants, particularly endangered wild species, is vital for biodiversity conservation, rational resource utilization, and breeding. This study proposes a resource evaluation model based on three key aspects: ecological adaptability, medicinal value, and ornamental value. It also reviews commonly employed research methods, such as the scoring method, analytic hierarchy process (AHP), and fuzzy mathematics. Looking forward, we highlight the importance of establishing fundamental evaluation indicators, integrating new technologies, leveraging big data, and strengthening evaluations for germplasm innovation and the protection of these multifunctional medicinal–ornamental plant resources in China.

Process mining aims at extracting information from event logs to capture the business process as it is being executed. Process mining is particularly useful in situations where events are recorded but there is no system enforcing people to work in a particular way. Consider for example a hospital where the diagnosis and treatment activities are recorded in the hospital information system, but where health-care professionals determine the \"careflow.\" Many process mining approaches have been proposed in recent years. However, in spite of many researchers' persistent efforts, there are still several challenging problems to be solved. In this paper, we focus on mining non-free-choice constructs, i.e., situations where there is a mixture of choice and synchronization. Although most real-life processes exhibit non-free-choice behavior, existing algorithms are unable to adequately deal with such constructs. Using a Petri-net-based representation, we will show that there are two kinds of causal dependencies between tasks, i.e., explicit and implicit ones. We propose an algorithm that is able to deal with both kinds of dependencies. The algorithm has been implemented in the ProM framework and experimental results shows that the algorithm indeed significantly improves existing process mining techniques. [PUBLICATION ABSTRACT]

Migrating foreign bodies of penis: a case report and literature review

Background
Only a few cases have been reported about active foreign body implantation in the cavernous body of the penis.
Case presentation
A 47-year-old man inserted two needles from the glans penis into the bilateral penile sponge body. Subsequently, two needles migrated through the penile cavernous body into the pelvic cavity. Attempts to remove the needles through the penis were unsuccessful. Eventually, after a duration exceeding one month, the displaced needles were removed in stages from the buttocks.
Conclusion
A few cases of intracavernosal-injection-therapy-associated needle breakage and retention have been reported globally. And this is the first case in China documenting the migration of foreign bodies within the penile region. In this condition, it is of utmost importance to engage the expertise of experienced andrologists to minimize the risk of excessive manipulation, thereby ensuring that inadvertent deep penetration of the needle into the penile tissue is prevented. In case the foreign body has migrated deeper into the tissues and the patient does not exhibit any specific symptoms or risks of macrovascular injury-related bleeding, close surveillance of its movement can be implemented. Surgical intervention can be initiated once the foreign body has reached a suitable position. Moreover, a psychiatric evaluation should be recommended for patient to discover any underlying mental health disorders.

A Business Process Analysis Methodology Based on Process Mining for Complaint Handling Service Processes

To improve the service quality of complaint handling service in a manufacturing company, it is key to analyze the business processes. Process mining is quite a useful approach to diagnose complaint handling service process problems, such as bottlenecks and deviations. However, the current business process analysis methodologies based on process mining mainly focus on operational process analysis and neglect other system level analysis. In this study, we introduce the method of Accimap from the discipline of accident analysis to analyze the diagnosis results of process mining. By creating a complaint handling service process management Accimap model, the process mining results analysis can be carried out across different system levels. A case study in a big manufacturing company in China is implemented to verify our approach. In the case study, 42 complaint handling process management factors are identified and the complaint handling process management Accimap model is created. The testing results by Rasmussen’s seven predictions in his risk management framework show that Accimap method presents a systematic approach to analyze the process diagnosis results based on process mining.

Readiness for return to work and its influencing factors among head and neck cancer patients: a cross-sectional study

Purpose
This study aims to investigate the Readiness for Return-to-Work (RRTW) of patients with head and neck tumours and to analyse the relationships among self-efficacy, disease uncertainty, psychosocial adaptation, and RRTW in head and neck cancer (HNC) patients.
Methods
A cross-sectional study was conducted with 259 HNC patients with a discharge length of ≥1 month at a tertiary hospital in Liaoning Province. The research tools included a self-designed general information questionnaire, the Readiness for Return-to-Work (RRTW) Scale, the General Self-Efficacy Scale (GSES), the Mishel Uncertainty in Illness Scale (MUIS), and the Self-Reporting Psychosocial Adjustment to Illness Scale (PAIS-SR). Descriptive statistical analysis, the rank sum test, Spearman correlation analysis, and ordered multiple and dichotomous logistic regression analyses were used.
Results
The overall RRTW among HNC patients was low (41.9%). HNC patients who did not return to work were mainly in the precontemplation stage (38.1%) and contemplation stage (29.9%). HNC patients who returned to work were mainly in the active maintenance stage (64.2%). Children’s status (OR = 0.218, 95% CI 0.068–0.703), self-efficacy (OR = 1.213, 95% CI 1.012–1.454), unpredictability (OR = 0.845, 95% CI 0.720–0.990), occupational environment (OR = 0.787, 95% CI 0.625–0.990), and family environment (OR = 0.798, 95% CI 0.643–0.990) influence the RRTW of HNC patients who have not returned to work. Educational level (OR = 62.196, 95% CI 63.307–68.567), children’s status (OR = 0.058, 95% CI 1.004–2.547), self-efficacy (OR = 1.544, 95% CI 3.010–8.715), unpredictability (OR = 0.445, 95% CI 1.271–2.280), and psychological status (OR = 0.340, 95% CI 1.141–2.401) influence the RRTW of HNC patients who have returned to work.
Conclusion
Children’s status, education level, self-efficacy, illness uncertainty, and psychosocial adjustment are crucial to RRTW. This study provides a theoretical basis for formulating intervention measures aimed at improving the RRTW of patients.
