A Systematic Review of Social Contact Surveys to Inform Transmission Models of Close-contact Infections

BACKGROUND:Researchers increasingly use social contact data to inform models for infectious disease spread with the aim of guiding effective policies about disease prevention and control. In this article, we undertake a systematic review of the study design, statistical analyses, and outcomes of the many social contact surveys that have been published.
METHODS:We systematically searched PubMed and Web of Science for articles regarding social contact surveys. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines as closely as possible.
RESULTS:In total, we identified 64 social contact surveys, with more than 80% of the surveys conducted in high-income countries. Study settings included general population (58%), schools or universities (37%), and health care/conference/research institutes (5%). The largest number of studies did not focus on a specific age group (38%), whereas others focused on adults (32%) or children (19%). Retrospective (45%) and prospective (41%) designs were used most often with 6% using both for comparison purposes. The definition of a contact varied among surveys, e.g., a nonphysical contact may require conversation, close proximity, or both. We identified age, time schedule (e.g., weekday/weekend), and household size as relevant determinants of contact patterns across a large number of studies.
CONCLUSIONS:We found that the overall features of the contact patterns were remarkably robust across several countries, and irrespective of the study details. By considering the most common approach in each aspect of design (e.g., sampling schemes, data collection, definition of contact), we could identify recommendations for future contact data surveys that may be used to facilitate comparison between studies.

Quantifying the impact of physical distance measures on the transmission of COVID-19 in the UK

Background
To mitigate and slow the spread of COVID-19, many countries have adopted unprecedented physical distancing policies, including the UK. We evaluate whether these measures might be sufficient to control the epidemic by estimating their impact on the reproduction number (

, the average number of secondary cases generated per case).
Methods
We asked a representative sample of UK adults about their contact patterns on the previous day. The questionnaire was conducted online via email recruitment and documents the age and location of contacts and a measure of their intimacy (whether physical contact was made or not). In addition, we asked about adherence to different physical distancing measures. The first surveys were sent on Tuesday, 24 March, 1 day after a “lockdown” was implemented across the UK. We compared measured contact patterns during the “lockdown” to patterns of social contact made during a non-epidemic period. By comparing these, we estimated the change in reproduction number as a consequence of the physical distancing measures imposed. We used a meta-analysis of published estimates to inform our estimates of the reproduction number before interventions were put in place.
Results
We found a 74% reduction in the average daily number of contacts observed per participant (from 10.8 to 2.8). This would be sufficient to reduce
R

from 2.6 prior to lockdown to 0.62 (95% confidence interval [CI] 0.37–0.89) after the lockdown, based on all types of contact and 0.37 (95% CI = 0.22–0.53) for physical (skin to skin) contacts only.
Conclusions
The physical distancing measures adopted by the UK public have substantially reduced contact levels and will likely lead to a substantial impact and a decline in cases in the coming weeks. However, this projected decline in incidence will not occur immediately as there are significant delays between infection, the onset of symptomatic disease, and hospitalisation, as well as further delays to these events being reported. Tracking behavioural change can give a more rapid assessment of the impact of physical distancing measures than routine epidemiological surveillance.

Individual social contact data and population mobility data as early markers of SARS-CoV-2 transmission dynamics during the first wave in Germany—an analysis based on the COVIMOD study

Background
The effect of contact reduction measures on infectious disease transmission can only be assessed indirectly and with considerable delay. However, individual social contact data and population mobility data can offer near real-time proxy information. The aim of this study is to compare social contact data and population mobility data with respect to their ability to reflect transmission dynamics during the first wave of the SARS-CoV-2 pandemic in Germany.
Methods
We quantified the change in social contact patterns derived from self-reported contact survey data collected by the German COVIMOD study from 04/2020 to 06/2020 (compared to the pre-pandemic period from previous studies) and estimated the percentage mean reduction over time. We compared these results as well as the percentage mean reduction in population mobility data (corrected for pre-pandemic mobility) with and without the introduction of scaling factors and specific weights for different types of contacts and mobility to the relative reduction in transmission dynamics measured by changes in
R
values provided by the German Public Health Institute.
Results
We observed the largest reduction in social contacts (90%, compared to pre-pandemic data) in late April corresponding to the strictest contact reduction measures. Thereafter, the reduction in contacts dropped continuously to a minimum of 73% in late June. Relative reduction of infection dynamics derived from contact survey data underestimated the one based on reported
R
values in the time of strictest contact reduction measures but reflected it well thereafter. Relative reduction of infection dynamics derived from mobility data overestimated the one based on reported
R
values considerably throughout the study. After the introduction of a scaling factor, specific weights for different types of contacts and mobility reduced the mean absolute percentage error considerably; in all analyses, estimates based on contact data reflected measured
R
values better than those based on mobility.
Conclusions
Contact survey data reflected infection dynamics better than population mobility data, indicating that both data sources cover different dimensions of infection dynamics. The use of contact type-specific weights reduced the mean absolute percentage errors to less than 1%. Measuring the changes in mobility alone is not sufficient for understanding the changes in transmission dynamics triggered by public health measures.

Social contact patterns and implications for infectious disease transmission – a systematic review and meta-analysis of contact surveys

Infectious diseases, particularly those caused by airborne pathogens like SARS-CoV-2, spread by social contact, and understanding how people mix is critical in controlling outbreaks. To explore these patterns, researchers typically carry out large contact surveys. Participants are asked for personal information (such as gender, age and occupation), as well as details of recent social contacts, usually those that happened in the last 24 hours. This information includes, the age and gender of the contact, where the interaction happened, how long it lasted, and whether it involved physical touch. These kinds of surveys help scientists to predict how infectious diseases might spread. But there is a problem: most of the data come from high-income countries, and there is evidence to suggest that social contact patterns differ between places. Therefore, data from these countries might not be useful for predicting how infections spread in lower-income regions. Here, Mousa et al. have collected and combined data from 27 contact surveys carried out before the COVID-19 pandemic to see how baseline social interactions vary between high- and lower-income settings. The comparison revealed that, in higher-income countries, the number of daily contacts people made decreased with age. But, in lower-income countries, younger and older individuals made similar numbers of contacts and mixed with all age groups. In higher-income countries, more contacts happened at work or school, while in low-income settings, more interactions happened at home and people were also more likely to live in larger, intergenerational households. Mousa et al. also found that gender affected how long contacts lasted and whether they involved physical contact, both of which are key risk factors for transmitting airborne pathogens. These findings can help researchers to predict how infectious diseases might spread in different settings. They can also be used to assess how effective non-medical restrictions, like shielding of the elderly and workplace closures, will be at reducing transmissions in different parts of the world.

Social contact patterns during the COVID-19 pandemic in 21 European countries – evidence from a two-year study

Background
Most countries have enacted some restrictions to reduce social contacts to slow down disease transmission during the COVID-19 pandemic. For nearly two years, individuals likely also adopted new behaviours to avoid pathogen exposure based on personal circumstances. We aimed to understand the way in which different factors affect social contacts – a critical step to improving future pandemic responses.
Methods
The analysis was based on repeated cross-sectional contact survey data collected in a standardized international study from 21 European countries between March 2020 and March 2022. We calculated the mean daily contacts reported using a clustered bootstrap by country and by settings (at home, at work, or in other settings). Where data were available, contact rates during the study period were compared with rates recorded prior to the pandemic. We fitted censored individual-level generalized additive mixed models to examine the effects of various factors on the number of social contacts.
Results
The survey recorded 463,336 observations from 96,456 participants. In all countries where comparison data were available, contact rates over the previous two years were substantially lower than those seen prior to the pandemic (approximately from over 10 to < 5), predominantly due to fewer contacts outside the home. Government restrictions imposed immediate effect on contacts, and these effects lingered after the restrictions were lifted. Across countries, the relationships between national policy, individual perceptions, or personal circumstances determining contacts varied.
Conclusions
Our study, coordinated at the regional level, provides important insights into the understanding of the factors associated with social contacts to support future infectious disease outbreak responses.

Social contact patterns in South Korea: an analysis of a survey conducted in 2023-2024

Background
Understanding social contact patterns is fundamental to the study of infectious disease transmission. However, in South Korea, detailed social contact data have not been publicly available. While global research on social contact patterns has expanded, there remains a critical need for more context-specific data in South Korea.
Methods
We conducted a social contact survey over two distinct weeks covering various time periods, including school vacations and national holidays. Participants provided details such as the location, duration, frequency, and type of close contact, as well as information on the contact person’s age, sex, residential area and relationship with the participant. We analyzed the data using summary statistics and the Bayesian linear mixed model.
Results
A total of 1,987 participants recorded 133,776 contacts over two weeks, averaging 4.81 contacts per participant per day. The average number of contacts per day varied by age, household size, and time period. Contacts were highest in the age group 5-19, lowest in the age group 20-29, and then gradually increased up to the age group 70+. Contacts also increased with household size. Weekdays during the school semester showed the highest number of contacts, followed by weekdays during vacations, the Lunar New Year holidays, and weekends. Contact patterns differed notably by period; during the Lunar New Year holidays, closed contacts with extended family members and, therefore, subnational social mixing were enhanced.
Conclusion
Our analyses across different time periods revealed significant and some unique variations of social contact patterns in South Korea. These findings can improve our understanding of infectious disease transmission in South Korea and will be useful for tailoring regional epidemiological models.

Dynamics of contact behaviour by self-reported COVID-19 vaccination and infection status during the COVID-19 pandemic in Germany: an analysis of two large population-based studies

Background
Contact behaviour is crucial to assess and predict transmission of respiratory pathogens like SARS-CoV-2. Contact behaviour has traditionally been assessed in cross-sectional surveys and not as part of longitudinal population-based studies which simultaneously measure infection frequency and vaccination coverage. During the COVID-19 pandemic, several studies assessed contact behaviour over longer periods and correlated this to data on immunity. This can inform future dynamic modelling. Here, we assess how contact behaviour varied based on SARS-CoV-2 infection or vaccination status in two large population-based studies in Germany during 2021.
Methods
We assessed direct encounters, separated into household and non-household contacts, in participants of MuSPAD (
n
= 12,641), a population-based cohort study, and COVIMOD (
n
= 31,260), a longitudinal contact survey. We calculated mean numbers of reported contacts and fitted negative binomial mixed-effects models to estimate the impact of immunity status, defined by vaccination or previous infection, on contact numbers; logistic mixed-effects models were used to examine the relationship between contact behaviour and seropositivity due to infection.
Results
Contact numbers varied over the course of the pandemic from 7.6 to 10.8 per 24 h in MuSPAD and 2.1 to 3.1 per 24 h in COVIMOD. The number of non-household contacts was higher in participants who reported previous infections and vaccinations (contact ratio (CR) MuSPAD: 1.22 (95%CI 0.94–1.60); COVIMOD: 1.35 (CI 1.12–1.62)) compared to unvaccinated and uninfected individuals. Non-household contact numbers were also higher in fully vaccinated participants (MUSPAD: CR 1.15 (CI 1.05–1.26); COVIMOD: 1.43 (CI 1.32–1.56)) compared to unvaccinated individuals. Compared to individuals without household contacts, the odds for seropositivity due to infection were higher among MuSPAD individuals with three or more household contacts (odds ratio (OR) 1.54 (CI 1.12–2.13)) and eleven or more non-household contacts (OR 1.29 (CI 1.01–1.65)).
Conclusions
Different contact behaviours based on infection and/or vaccination status suggest that public health policies targeting immunity status may influence the contact behaviour of those affected. A combined assessment of self-reported contacts, infections, and vaccinations as well as laboratory-confirmed serostatus in the population can support modelling of the spread of infections. This could help target containment policies and evaluate the impact of public health measures.

Socio-demographic characteristics associated with COVID-19 vaccination uptake in Switzerland: longitudinal analysis of the CoMix study

Background
Vaccination is an effective strategy to reduce morbidity and mortality from coronavirus disease 2019 (COVID-19). However, the uptake of COVID-19 vaccination has varied across and within countries. Switzerland has had lower levels of COVID-19 vaccination uptake in the general population than many other high-income countries. Understanding the socio-demographic factors associated with vaccination uptake can help to inform future vaccination strategies to increase uptake.
Methods
We conducted a longitudinal online survey in the Swiss population, consisting of six survey waves from June to September 2021. Participants provided information on socio-demographic characteristics, history of testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), social contacts, willingness to be vaccinated, and vaccination status. We used a multivariable Poisson regression model to estimate the adjusted rate ratio (aRR) and 95% confidence intervals (CI) of COVID-19 vaccine uptake.
Results
We recorded 6,758 observations from 1,884 adults. For the regression analysis, we included 3,513 observations from 1,883 participants. By September 2021, 600 (75%) of 806 study participants had received at least one vaccine dose. Participants who were older, male, and students, had a higher educational level, household income, and number of social contacts, and lived in a household with a medically vulnerable person were more likely to have received at least one vaccine dose. Female participants, those who lived in rural areas and smaller households, and people who perceived COVID-19 measures as being too strict were less likely to be vaccinated. We found no significant association between previous SARS-CoV-2 infections and vaccination uptake.
Conclusions
Our results suggest that socio-demographic factors as well as individual behaviours and attitudes played an important role in COVID-19 vaccination uptake in Switzerland. Therefore, appropriate communication with the public is needed to ensure that public health interventions are accepted and implemented by the population. Tailored COVID-19 vaccination strategies in Switzerland that aim to improve uptake should target specific subgroups such as women, people from rural areas or people with lower socio-demographic status.

Contact patterns of older adults with and without frailty in the Netherlands during the COVID-19 pandemic

Background
During the COVID-19 pandemic, social distancing measures were imposed to protect the population from exposure, especially older adults and people with frailty, who have the highest risk for severe outcomes. These restrictions greatly reduced contacts in the general population, but little was known about behaviour changes among older adults and people with frailty themselves. Our aim was to quantify how COVID-19 measures affected the contact behaviour of older adults and how this differed between older adults with and without frailty.
Methods
In 2021, a contact survey was carried out among people aged 70 years and older in the Netherlands. A random sample of persons per age group (70–74, 75–79, 80–84, 85–89, and 90 +) and gender was invited to participate, either during a period with stringent (April 2021) or moderate (October 2021) measures. Participants provided general information on themselves, including their frailty, and they reported characteristics of all persons with whom they had face-to-face contact on a given day over the course of a full week.
Results
In total, 720 community-dwelling older adults were included (overall response rate of 15%), who reported 16,505 contacts. During the survey period with moderate measures, participants without frailty had significantly more contacts outside their household than participants with frailty. Especially for females, frailty was a more informative predictor of the number of contacts than age. During the survey period with stringent measures, participants with and without frailty had significantly lower numbers of contacts compared to the survey period with moderate measures. The reduction of the number of contacts was largest for the eldest participants without frailty. As they interact mostly with adults of a similar high age who are likely frail, this reduction of the number of contacts indirectly protects older adults with frailty from SARS-CoV-2 exposure.
Conclusions
The results of this study reveal that social distancing measures during the COVID-19 pandemic differentially affected the contact patterns of older adults with and without frailty. The reduction of contacts may have led to the direct protection of older adults in general but also to the indirect protection of older adults with frailty.

The Effect of Prior Police Contact on Victimization Reporting

Objectives
This study explores the association between past-year face-to-face contact with the police and subsequent victimization reporting. It also examines whether this relationship depends on the type of encounter (citizen-initiated contacts, routine vehicle stops, invasive encounters associated with being a suspect), its perceived justness, or victim characteristics. Among victims who did not notify the police, the reasons behind this decision are assessed to understand the mechanisms through which police encounters are related to reporting.
Methods
This research is the first to use longitudinal data that link the 2002, 2008, and 2011 Police Public Contact Surveys to the 2002–2014 National Crime Victimization Surveys. Multivariate logistic regression is used to examine the effect of prior contact on police notification among victims of personal (N = 1073) and household (N = 11,433) crimes.
Results
Prior contact with the police has no main effect on the reporting of personal crimes; however, the negative effects of police-initiated and unjust contact are amplified for the poor and African Americans. The reporting of household crimes varies based on prior police experiences and whether they were viewed as just. Personal crime victims with invasive contact are more likely than other contact groups to attribute non-reporting to fear of reprisal.
Conclusions
Using national data tells a complex story about how race/ethnicity, poverty, and recent experiences with the police interact to shape victims’ behavior. The intricacies of these findings suggest that efforts to increase reporting may need to be multifaceted and address victims’ concerns about safety and justness.
