Inventing destiny : cultural explorations of US expansion
\"Conventional studies of US expansion have viewed the topic from political, diplomatic, economic, and military perspectives. In this collection, an interdisciplinary cast of scholars examine the subject from a variety of cultural perspectives, showing how narratives of empire and conquest formed in both low and high cultures. Many of the chapters will focus on the less remembered actors and outliers - anti-expansionists, Native Americans, Anglo-American women, and non-national expansionists - who significantly complicated the narratives of empire. The contributors examine a variety of source materials such as artwork, literature, and geospatial analysis alongside fresh readings of traditional historical texts. P8 In silico assessment of vascular injury risk in redo-TAVI: impact of valve expansion strategies

Redo-transcatheter aortic valve implantation (redo-TAVI) is an emerging solution for failed transcatheter heart valves (THVs) in patients with aortic stenosis who outlive their initial implants. While early outcomes are promising, complications such as vascular injury (VI) remain a concern, particularly due to high radial forces during valve deployment. The choice of THV type, deployment strategy, and anatomical features such as calcifications influence VI risk, potentially leading to vessel rupture and bleeding.1 This study aims to identify biomechanical markers associated with VI using in silico simulations of redo-TAVI under varying expansion conditions. A 23 mm Sapien 3 THV was deployed as both index and redo THV in a patient-specific aortic root. Three expansion levels, under, nominal, and over-expansion, were compared in terms of stresses and strains. The aortic root and THV leaflets were modelled as hyperelastic (3rd-order Ogden), with elasto-plastic properties for the stent, catheter tip, and calcifications, and elastic properties for the balloon and skirting.2 Deployment was simulated using Abaqus/Explicit in two dynamic steps: leaflet opening via catheter displacement, followed by balloon inflation using fluid cavity interaction. General contact was defined with penalty friction and hard contact, and mass scaling kept the kinetic-to-internal energy ratio below 5–10% to maintain quasi-static conditions.Results showed over-expansion increased peak stresses in the aortic root, exceeding 2.5 MPa,3 particularly in calcified regions, highlighting elevated VI risk. This work establishes a computational framework for evaluating deployment-induced injury risk in redo-TAVI, with future directions including cohort expansion, damage modelling, and different THV combinations.References Mach M, et al. Background The super‐thin skin flap formed by skin and soft tissue expansion has large area and good ductility, so it can be used to repair skin defects. However, because the flap is thin, the blood flow in the dermis of the super‐thin expanded flap is weakened, and flap rupture and necrosis after secondary flap transfer may occur. Objective To compare the skin thickness difference between the expanded ultrathin flaps injected with concentrated growth factor (CGF) and the blank group or saline group. Methods From June 2021 to December 2023, 10 patients (44 sites) with large‐area scars or skin tumors were treated, and a single center half randomized controlled trial was conducted. The test site of expander implantation was divided into three groups: intradermal injection of CGF group, normal saline group and blank group. The same amount of expansion was performed every 1–2 weeks, and CGF or normal saline was injected into the dermis every 4 weeks, a total of three times. After 2–3 months of expansion, color Doppler ultrasound was used to measure the skin thickness of each group. Results Compared with the blank group, the skin thickness of CGF group was 1.75 ± 0.08 mm, and that of BLA blank group was 1.42 ± 0.07 mm, with statistically significant difference (p < 0.0001); In the other group, compared with the saline group, the skin thickness of the CGF group was 1.54 ± 0.08 mm, and the average skin thickness of the saline group was 1.40 ± 0.08 mm, with significant difference between the two groups (p = 0.0067). Conclusion CGF intradermal injection can increase the skin thickness of super‐thin skin flap in the process of soft tissue expansion, which is a safe and effective auxiliary method of skin expansion.

ObjectivesTo provide a higher degree of evidence on the efficacy of Miniscrew-Assisted Rapid Palatal Expansion (MARPE) in late adolescents and adults, thereby applying the Dutch Maxillary Expansion Device (D-MED).Materials and methodsD-MED was developed as an individualized, 3D-designed, and fabricated MARPE appliance supported by 4 palatal miniscrews. Patients from the age of 16 onwards with transverse maxillary deficiency were enrolled consecutively. Pre-expansion and immediate post-expansion CBCTs and intra-oral scans were acquired and measurements of skeletal, alveolar, and dental expansion as well as dental and periodontal side-effects were performed.ResultsThirty-four patients were enrolled (8 men, 26 women) with mean age 27.0 ± 9.4 years. A success rate of 94.1% was achieved (32/34 patients). The mean expansion duration, or mean observation time, was 31.7 ± 8.0 days. The mean expansion at the maxillary first molars (M1) and first premolars (P1) was 6.56 ± 1.70 mm and 4.19 ± 1.29 mm, respectively. The expansion was 60.4 ± 20.1% skeletal, 8.1 ± 27.6% alveolar, and 31.6 ± 20.1% dental at M1 and 92.2 ± 14.5% skeletal, 0.0 ± 18.6% alveolar, and 7.8 ± 17.7% dental at P1, which was both statistically (p < 0.001) and clinically significant. Buccal dental tipping (3.88 ± 3.92° M1; 2.29 ± 3.89° P1), clinical crown height increase (0.12 ± 0.31 mm M1; 0.04 ± 0.22 mm P1), and buccal bone thinning (− 0.31 ± 0.49 mm M1; − 0.01 ± 0.45 mm P1) were observed, while root resorption could not be evaluated.ConclusionsMARPE by application of D-MED manifested its efficacy in a prospective clinical setting, delivering a high amount of skeletal expansion with limited side-effects in late adolescents and adults.Clinical relevanceHigher quality evidence is supportive of MARPE as a safe and successful non-surgical treatment option for transverse maxillary deficiency.
