Impact of war and forced displacement on children’s mental health—multilevel, needs-oriented, and trauma-informed approaches

The infliction of war and military aggression upon children must be considered a violation of their basic human rights and can have a persistent impact on their physical and mental health and well-being, with long-term consequences for their development. Given the recent events in Ukraine with millions on the flight, this scoping policy editorial aims to help guide mental health support for young victims of war through an overview of the direct and indirect burden of war on child mental health. We highlight multilevel, need-oriented, and trauma-informed approaches to regaining and sustaining outer and inner security after exposure to the trauma of war. The impact of war on children is tremendous and pervasive, with multiple implications, including immediate stress-responses, increased risk for specific mental disorders, distress from forced separation from parents, and fear for personal and family’s safety. Thus, the experiences that children have to endure during and as consequence of war are in harsh contrast to their developmental needs and their right to grow up in a physically and emotionally safe and predictable environment. Mental health and psychosocial interventions for war-affected children should be multileveled, specifically targeted towards the child’s needs, trauma-informed, and strength- and resilience-oriented. Immediate supportive interventions should focus on providing basic physical and emotional resources and care to children to help them regain both external safety and inner security. Screening and assessment of the child’s mental health burden and resources are indicated to inform targeted interventions. A growing body of research demonstrates the efficacy and effectiveness of evidence-based interventions, from lower-threshold and short-term group-based interventions to individualized evidence-based psychotherapy. Obviously, supporting children also entails enabling and supporting parents in the care for their children, as well as providing post-migration infrastructures and social environments that foster mental health. Health systems in Europe should undertake a concerted effort to meet the increased mental health needs of refugee children directly exposed and traumatized by the recent war in Ukraine as well as to those indirectly affected by these events. The current crisis necessitates political action and collective engagement, together with guidelines by mental health professionals on how to reduce harm in children either directly or indirectly exposed to war and its consequences.

Early childhood deprivation is associated with higher rates of neurodevelopmental and mental disorders in adulthood. The impact of childhood deprivation on the adult brain and the extent to which structural changes underpin these effects are currently unknown. To investigate these questions, we utilized MRI data collected from young adults who were exposed to severe deprivation in early childhood in the Romanian orphanages of the Ceauşescu era and then, subsequently adopted by UK families; 67 Romanian adoptees (with between 3 and 41 mo of deprivation) were compared with 21 nondeprived UK adoptees. Romanian adoptees had substantially smaller total brain volumes (TBVs) than nondeprived adoptees (8.6% reduction), and TBV was strongly negatively associated with deprivation duration. This effect persisted after covarying for potential environmental and genetic confounds. In whole-brain analyses, deprived adoptees showed lower right inferior frontal surface area and volume but greater right inferior temporal lobe thickness, surface area, and volume than the nondeprived adoptees. Right medial prefrontal volume and surface area were positively associated with deprivation duration. No deprivation-related effects were observed in limbic regions. Global reductions in TBV statistically mediated the observed relationship between institutionalization and both lower intelligence quotient (IQ) and higher levels of attention deficit/hyperactivity disorder symptoms. The deprivation-related increase in right inferior temporal volume seemed to be compensatory, as it was associated with lower levels of attention deficit/hyperactivity disorder symptoms. We provide compelling evidence that time-limited severe deprivation in the first years of life is related to alterations in adult brain structure, despite extended enrichment in adoptive homes in the intervening years.

Impact of process quality in early childhood education and care on academic outcomes: Longitudinal meta-analysis

This meta-analysis studies the association of pedagogical processes in early childhood care and education with outcomes in two academic domains: language and literacy as well as mathematics. It synthesizes evidence from 17 longitudinal studies in nine European countries with 16,461 children in regular center-based care spanning the period between ages 3 and 16. Results of a three-level meta-analysis provided small overall effect sizes for both global and domain-specific process quality, and indicated lasting associations with academic development over children's school career (ES = .11, Cohen's d = .22, and ES = .10, Cohen's d = .20). Effects varied by outcome domain, type of process measure, and differences in adopted analyses, with important implications for pedagogical practice and future research. (ZPID).

Acute stress affects interoception, but it remains unclear if this is due to activation of the sympatho-adreno-medullary (SAM) or hypothalamic–pituitary–adrenocortical axis. This study aimed to investigate the effect of SAM axis activation on interoceptive accuracy (IAcc). Central alpha2-adrenergic receptors represent a negative feedback mechanism of the SAM axis. Major depressive disorder and adverse childhood experiences (ACE) are associated with alterations in the biological stress systems, including central alpha2-adrenergic receptors. Here, healthy individuals with and without ACE as well as depressive patients with and without ACE (n = 114; all without antidepressant medication) were tested after yohimbine (alpha2-adrenergic antagonist) and placebo. We assessed IAcc and sensibility in a heartbeat counting task. Increases in systolic and diastolic blood pressure after yohimbine confirmed successful SAM axis activation. IAcc decreased after yohimbine only in the healthy group with ACE, but remained unchanged in all other groups (Group × Drug interaction). This effect may be due to selective upregulation of alpha2-adrenergic receptors after childhood trauma, which reduces capacity for attention focus on heartbeats. The sympathetic neural pathway including alpha2-adrenergic circuitries may be essential for mediating interoceptive signal transmission. Suppressed processing of physical sensations in stressful situations may represent an adaptive response in healthy individuals who experienced ACE.

Associations of Parenting Styles with Self-Esteem in Children and Adolescents: A Meta-Analysis

ObjectivesThe objective of the present meta-analysis was to integrate the available research on associations of parenting styles with self-esteem in children and adolescents.MethodsA systematic search in electronic databases (PSYCINFO, ERIC, Google Scholar, and PSYNDEX) and cross referencing identified 116 studies that were included in a random-effects meta-analysis.ResultsCross-sectional studies found small to moderate positive associations of authoritative parenting with self-esteem (r = 0.26; 95%-CI [0.24, 0.29]) while authoritarian (r = −0.18; 95%-CI [−0.21, −0.14]) and neglectful parenting (r = −0.18; 95%-CI [−0.23, −0.12]) were related to lower self-esteem in the offspring. A very small positive association of permissive parenting with self-esteem was observed in studies that defined permissiveness by low control and high warmth rather than only by low control (r = 0.07; 95%-CI [0.01, 0.12]). Cross-lagged analyses found evidence for child effects on change in authoritative (r = 0.13; 95%-CI [0.05, 0.21]) and neglectful parenting (r = −0.28; 95%-CI [−0.34, −0.22] but not on effects of parenting styles on change in self-esteem; however very few longitudinal studies were available. Few moderating effects of study characteristics were identified.ConclusionsWe conclude that correlations between parenting styles and child self-esteem cannot be interpreted as a pure effect of parenting styles and that more longitudinal research is urgently needed for testing potential bidirectional effects.

Cultural diversity climate and psychological adjustment at school - equality and inclusion versus cultural pluralism

Examined the cultural diversity climate at school and how it relates to acculturation orientations and psychological school adjustment of early adolescent immigrants. Specifically, the distinct role of two types of diversity policy is investigated, namely (a) fostering equality and inclusion and (b) acknowledging cultural pluralism. Longitudinal multilevel analyses based on 386 early adolescent immigrant students (mean age 10 years) in 44 ethnically heterogeneous classrooms in Germany revealed that the manifestations of both types of policies promote psychological school adjustment (i.e., better well-being and fewer psychological and behavioral problems) at the individual level. However, they differ in their effects on acculturation orientations. At the classroom level, equality and inclusion promote assimilation. Implications for research and educational practice are discussed. (ZPID).

Childhood Trauma in Schizophrenia: Current Findings and Research Perspectives

Schizophrenia is a severe neuropsychiatric disorder with persistence of symptoms throughout adult life in most of the affected patients. This unfavorable course is associated with multiple episodes and residual symptoms, mainly negative symptoms and cognitive deficits. The neural diathesis-stress model proposes that psychosocial stress acts on a pre-existing vulnerability and thus triggers the symptoms of schizophrenia. Childhood trauma is a severe form of stress that renders individuals more vulnerable to developing schizophrenia; neurobiological effects of such trauma on the endocrine system and epigenetic mechanisms are discussed. Childhood trauma is associated with impaired working memory, executive function, verbal learning, and attention in schizophrenia patients, including those at ultra-high risk to develop psychosis. In these patients, higher levels of childhood trauma were correlated with higher levels of attenuated positive symptoms, general symptoms, and depressive symptoms; lower levels of global functioning; and poorer cognitive performance in visual episodic memory end executive functions. In this review, we discuss effects of specific gene variants that interact with childhood trauma in patients with schizophrenia and describe new findings on the brain structural and functional level. Additive effects between childhood trauma and brain-derived neurotrophic factor methionine carriers on volume loss of the hippocampal subregions cornu ammonis (CA)4/dentate gyrus and CA2/3 have been reported in schizophrenia patients. A functional magnetic resonance imaging study showed that childhood trauma exposure resulted in aberrant function of parietal areas involved in working memory and of visual cortical areas involved in attention. In a theory of mind task reflecting social cognition, childhood trauma was associated with activation of the posterior cingulate gyrus, precuneus, and dorsomedial prefrontal cortex in patients with schizophrenia. In addition, decreased connectivity was shown between the posterior cingulate/precuneus region and the amygdala in patients with high levels of physical neglect and sexual abuse during childhood, suggesting that disturbances in specific brain networks underlie cognitive abilities. Finally, we discuss some of the questionnaires that are commonly used to assess childhood trauma and outline possibilities to use recent biostatistical methods, such as machine learning, to analyze the resulting datasets.

Childhood loneliness as a specific risk factor for adult psychiatric disorders

Loneliness is a major risk factor for both psychological disturbance and poor health outcomes in adults. This study aimed to assess whether
loneliness is associated with a long-term disruption in mental health that extends into adulthood.
This study is based on the longitudinal, community-representative Great Smoky Mountains Study of 1420 participants. Participants were assessed with the structured Child and Adolescent Psychiatric Assessment interview up to eight times in childhood (ages 9-16; 6674 observations; 1993-2000) for childhood loneliness, associated psychiatric comorbidities and childhood adversities. Participants were followed up four times in adulthood (ages 19, 21, 25, and 30; 4556 observations of 1334 participants; 1999-2015) with the structured Young Adult Psychiatric Assessment Interview for psychiatric anxiety, depression, and substance use outcomes.
Both self and parent-reported childhood loneliness were associated with adult self-reported anxiety and depressive outcomes. The associations remained significant when childhood adversities and psychiatric comorbidities were accounted for. There was no evidence for an association of childhood loneliness with adult substance use disorders. More associations were found between childhood loneliness and adult psychiatric symptoms than with adult diagnostic status.
Childhood loneliness is associated with anxiety and depressive disorders in young adults, suggesting that loneliness - even in childhood - might have long-term costs in terms of mental health. This study underscores the importance of intervening early to prevent loneliness and its sequelae over time.

Self-, other-, and dual-harm during adolescence: a prospective-longitudinal study of childhood risk factors and early adult correlates

Little is known about the childhood antecedents and adult correlates of adolescent dual-harm (i.e. co-occurring self- and other-harm). We examine the longitudinal associations between (a) social and psychological risk factors in childhood and adolescent dual-harm and (b) adolescent dual-harm and social and mental health impairments in early adulthood.
Participants (N = 1482) are from a prospective longitudinal community-representative study. Dual-, self-, and other-harm were self-reported at ages 13, 15, and 17. Social and psychological risk factors in childhood were assessed between 7 and 11; early adult correlates at age 20. Groups with dual-harm, self-harm only, other-harm only, and no harm were compared.
Between 13 and 17, 7.2% of adolescents reported dual-harm (self-harm only: 16.2%; other-harm only: 13.3%). Some childhood risk factors (e.g. sensation-seeking, parental divorce, victimization by peers) characterized all harm groups; others were common to the dual- and self-harm (anxiety/depressive symptoms, relational aggression) or dual- and other-harm groups only (low self-control, substance use, delinquency). Adolescents with dual-harm had reported more physical aggression and harsh parenting, and lower school bonding in childhood than any other group. In early adulthood, they reported more anxiety/depressive symptoms, psychopathy symptoms, homicidal ideations, delinquency, and victimization experiences than any other group.
Adolescent dual-harm follows psychological problems and social disconnection in childhood and signals risk of psychopathology and isolation in early adulthood. To curb the burden from dual-harm, interventions must target adolescents, families, peer networks, and school environments. Differentiating youth with dual-harm from those with single-harm is important for developing personalized treatments.

An Update on Secular Trends in Physical Fitness of Children and Adolescents from 1972 to 2015: A Systematic Review

Background
There is evidence that physical fitness of children and adolescents (particularly cardiorespiratory endurance) has declined globally over the past decades. Ever since the first reports on negative trends in physical fitness, efforts have been undertaken by for instance the World Health Organization (WHO) to promote physical activity and fitness in children and adolescents. Therefore, it is timely to re-analyze the literature to examine whether previous reports on secular declines in physical fitness are still detectable or whether they need to be updated.
Objectives
The objective of this systematic review is to provide an ‘update’ on secular trends in selected components of physical fitness (i.e., cardiorespiratory endurance, relative muscle strength, proxies of muscle power, speed) in children and adolescents aged 6–18 years.
Data Sources
A systematic computerized literature search was conducted in the electronic databases PubMed and Web of Science to locate studies that explicitly reported secular trends in physical fitness of children and adolescents.
Study Eligibility Criteria
Studies were included in this systematic review if they examined secular trends between at least two time points across a minimum of 5 years. In addition, they had to document secular trends in any measure of cardiorespiratory endurance, relative muscle strength, proxies of muscle power or speed in apparently healthy children and adolescents aged 6–18 years.
Study Appraisal and Synthesis Methods
The included studies were coded for the following criteria: nation, physical fitness component (cardiorespiratory endurance, relative muscle strength, proxies of muscle power, speed), chronological age, sex (boys vs. girls), and year of assessment. Scores were standardized (i.e., converted to
z
scores) with sample-weighted means and standard deviations, pooled across sex and year of assessment within cells defined by study, test, and children’s age.
Results
The original search identified 524 hits. In the end, 22 studies met the inclusion criteria for review. The observation period was between 1972 and 2015. Fifteen of the 22 studies used tests for cardiorespiratory endurance, eight for relative muscle strength, eleven for proxies of muscle power, and eight for speed. Measures of cardiorespiratory endurance exhibited a large initial increase and an equally large subsequent decrease, but the decrease appears to have reached a floor for all children between 2010 and 2015. Measures of relative muscle strength showed a general trend towards a small increase. Measures of proxies of muscle power indicated an overall small negative quadratic trend. For measures of speed, a small-to-medium increase was observed in recent years.
Limitations
Biological maturity was not considered in the analysis because biological maturity was not reported in most included studies.
Conclusions
Negative secular trends were particularly found for cardiorespiratory endurance between 1986 and 2010–12, irrespective of sex. Relative muscle strength and speed showed small increases while proxies of muscle power declined. Although the negative trend in cardiorespiratory endurance appears to have reached a floor in recent years, because of its association with markers of health, we recommend further initiatives in PA and fitness promotion for children and adolescents. More specifically, public health efforts should focus on exercise that increases cardiorespiratory endurance to prevent adverse health effects (i.e., overweight and obesity) and muscle strength to lay a foundation for motor skill learning.
