Purpose
Posterior horn meniscal tears are commonly found in conjunction with anterior cruciate ligament (ACL) injury. Some believe tears in the posterior meniscocapsular zone, coined ramp lesions, are important to knee stability. The purpose of this study was to determine whether pre-operative MRI evaluation was able to accurately and reproducibly identify ramp lesions.
Methods
Three blinded reviewers assessed MRIs twice for the presence of ramp lesions in patients undergoing ACL reconstruction. Sensitivity, specificity, negative predictive value, and positive predictive value for MRI were calculated based on arthroscopic diagnosis of a ramp lesion. Intra-class correlation coefficient was calculated to assess intra- and interobserver reliability of the MRI assessment between the three examiners. Significance was set at

Results
Ninety patients met inclusion criteria (45 males, 45 females, mean age 28.0 years). Thirteen of these patients had arthroscopy-confirmed ramp lesions, while the other 77 had other meniscal pathology. Sensitivity of detecting a ramp lesion on MRI ranged from 53.9 to 84.6%, while specificity was 92.3–98.7%. Negative predictive value was 91.1–97.4%, while positive predictive value was 50.0–90.0%. Inter-rater reliability between three reviewers was moderate at 0.56. The observers had excellent intra-rater reliability ranging from 0.75 to 0.81.
Conclusions
This study demonstrates high sensitivity and excellent specificity in detecting meniscal ramp lesions on MRI. Ramp lesions are likely more common and may have greater clinical implications than previously appreciated; the outcomes of untreated lesions must be investigated. Pre-operative identification of ramp lesions may aid clinicians in surgical planning and patient education to improve outcomes by addressing pathology which may have otherwise been missed.

Purpose
This study reported the outcomes of locked bucket-handle medial meniscal tear (BHMMT) repairs using an arthroscopic posterior approach during anterior cruciate ligament (ACL) reconstruction.
Methods
Between 2011 and 2014, 48 patients with BHMMTs and ACL tears who met the eligibility criteria were enrolled in the present study. BHMMTs were assessed using a posterolateral transseptal portal and repaired using a posteromedial portal. Transportal ACL reconstruction was performed using hamstrings autograft. Patients were assessed based on their IKDC and Lysholm scores and Tegner activity level. Meniscal healing was clinically evaluated based on the absence of swelling, joint line tenderness, locking, and catching; McMurray test results; and the need for meniscectomy.
Results
According to follow-up assessments, the average IKDC and Lysholm scores improved significantly after 3–5 years (

Conclusion
Excellent clinical outcomes were obtained when locked BHMMTs were repaired using an all-inside suture technique that employed posteromedial and posterolateral transseptal portals.

Double posteromedial portals versus single posteromedial portal for arthroscopic management of popliteal cysts

Background
As a common disease in orthopedic clinics, popliteal cysts often coexist with intra-articular lesions. Compared with traditional open surgery, arthroscopic treatment of popliteal cysts is less traumatic, and intra-articular lesions can be treated. The ‘one-way valve’ mechanism of the popliteal cyst can be removed by expanding the communication between the articular cavity and the cyst to avoid cyst recurrence. In terms of arthroscopic techniques, the comparison of clinical effects between the double posteromedial portal (DPP) and single posteromedial portal (SPP) has rarely been studied. The purpose of this retrospective study was to compare the clinical effects of DPP and SPP.
Methods
A total of 46 consecutive patients with symptomatic popliteal cysts who underwent arthroscopic treatment were included in this study and followed for approximately 1 year. All patients were divided into two groups according to the arthroscopic portals (DPP group and SPP group). The cyst size, Lysholm score and Rauschening–Lindgren (R–L) grade were evaluated before the operation for all patients, and the intra-articular lesions, operative time and complications were recorded after operation. At the last follow-up, the Lysholm score and R–L grade were recorded, and magnetic resonance imaging was used to evaluate the outcome of the cyst. The clinical data of the two groups was statistically compared and analyzed.
Results
There were no significant differences in preoperative cyst size, Lysholm score or R–L grade between the two groups (

> 0.05). The operation time of the DPP group (67.52 ± 18.23 min) was longer than that of the SPP group (55.95 ± 16.40 min) (
P
= 0.030), but the recurrence rate of cysts in the DPP group (0%) was obviously lower than that in the SPP group (19.0%) (

= 0.046). There were no significant differences in the Lysholm score, R–L grade or complication rate between the two groups at the last follow-up (

> 0.05).
Conclusion
Arthroscopic treatment of popliteal cysts using double posteromedial portals was a safe and effective surgical method.

Purpose
This study investigates the effects of the all-inside repair of posterosuperior popliteomeniscal fascicle (PMF) on lateral meniscus stabilization using a posterior arthroscopic approach.
Methods
Between 2015 and 2018, 17 patients with hypermobile lateral meniscus (HLM) underwent posterior knee arthroscopy for PMF repair. The all-inside repair was performed through posteromedial transseptal and posterolateral portals using a suture hook technique. Patients were clinically assessed based on IKDC and Lysholm scores.
Results
Both IKDC and Lysholm scores improved significantly after an average follow-up of 3.5 years (

< 0.001). No patients underwent reoperation, and no complications associated with posterior knee arthroscopy were reported.
Conclusion
The all-inside suture hook technique using posterolateral and posteromedial transseptal portals fixes HLM with excellent IKDC and Lysholm scores.

The use of posteromedial portal for arthroscopic treatment of synovial chondromatosis of the knee: a case report

Background
The synovial chondromatosis is an uncommon proliferative metaplastic process of the synovial cells that can develop in any synovial joint. An isolated primary chondromatosis of the posterior compartment of the knee is uncommon and few cases are reported in literature. Our purpose is to describe a rare case of primary chondromatosis of the knee posterior compartment and report the arthroscopic loose bodies excision through a difficult posteromedial portal, avoiding the use of the accessory posterior portal, most commonly reported for approaching this disease.
Case presentation
We report a rare case of a 35-year-old Caucasian male patient with diagnosis of chondromatosis of the posterior knee compartment. The radiographs showed multiple loose bodies of the posterior compartment. The MRI revealed minimal synovial hypertrophy areas, multiple osteophytes in the intercondylar notch, and loose bodies in the posteromedial compartment. The CT allowed us to assess the bony structures, the morphology of the intercondylar notch, and the presence osteophytes of the medial and lateral femoral condyles. The CT images were crucial to plan how to reach the posterior compartments of the knee through a trans-notch passage. The patient underwent arthroscopic surgery using anteromedial, anterolateral, and posteromedial portals. The tunneling through the intercondylar osteophytes was performed to allow the arthroscope to pass trans-notch. To avoid additional accessory posterior portals, we used a 70° arthroscope to better explore the posterior knee compartment. The cartilage-like bodies were removed and synovectomy of the inflamed areas was performed. The clinical and radiological follow-up was 12 months and the patient showed excellent clinical outcomes, returning to his activities of daily living and sport activity.
Conclusion
Our case report highlights the importance of the arthroscopic approach to treat synovial chondromatosis, despite the involvement of the posterior compartment of the knee. An optimal preoperative imaging allows to plan for the proper surgical procedure even in patients with severe osteoarthritis. Moreover, the adoption of an intercondylar notch tunneling and a 70° arthroscope can help surgeons to better explore the posterior knee compartment, avoiding an accessory posterior trans-septal portal. Therefore, a synovectomy of the inflamed foci may be useful to prevent recurrence.

Arthroscopic treatment of popliteal cyst using a figure-of-four position and double posteromedial portals

Purpose
The purpose of this study was to introduce a modified arthroscopic treatment technique for popliteal cyst and hypothesize that this modified technique would provide good clinical efficacy and low recurrence rate.
Methods
From January 2013 to January 2017, 34 patients with symptomatic popliteal cysts were treated with our technique. A figure-of-four position and double posteromedial portals were used to achieve adequate enlargement of the posteromedial valvular opening between the cyst and the joint cavity and complete excision of the cyst wall. MRI was used to detect the recurrence of the popliteal cyst, and the Rauschning and Lindgren score was recorded to evaluate the clinical outcome.
Results
All patients were followed up with a mean period of 14.8 months (range, 12 to 36 months). Associated intra-articular lesions were found and treated in all cases. Degenerative cartilage damage was the most common pathology, which affected 23 (67.6%) of the cases. The Rauschning and Lindgren score improved significantly after surgery, and no evidence of recurrence was found from MRI in any case.
Conclusions
Our modified arthroscopic treatment technique, using a figure-of-four position and double posteromedial portals, is effective and safe for treating popliteal cyst.

Arthroscopic removal of loose bodies using the accessory portals in the difficult locations of the knee: a case series and technical note

Background
It is often technically demanding to find and remove loose bodies in several difficult locations like the popliteus hiatus and posterior compartment arthroscopically. We aim to present the technical aspects of establishing some special accessory portals to achieve arthroscopic removal of the loose bodies in these locations.
Methods
From September 2010 to July 2017, 76 patients underwent removal of loose bodies in the popliteus hiatus and posterior compartment arthroscopically using some special accessory portal techniques. An auxiliary extreme lateral approach was established to remove loose bodies in the popliteus hiatus; a double-posteromedial portal was applied to handle loose body removal in the posteromedial compartment, and the posterior trans-septal portal was needed for loose body removal in the posterolateral compartment. Functional outcomes were evaluated using Lysholm score, Tegner score, and International Knee Documentation Committee (IKDC) score, respectively.
Results
Seventy-six patients (24 males and 52 females, average age 54.9 ± 11.4) finished the follow-up visit at 3 weeks after surgery. There was no statistically significant difference among the three groups in demographics. All the patients were performed following the special technique. According to a comparison of knee joint scores before and after surgery, all the patients obtained good prognosis using some special accessory portals in loose body removal.
Conclusions
With the help of the above accessory portals under endoscopic visualization, loose bodies in the popliteus hiatus and posterior compartment of the knee can be safely and effectively removed.

The relationship of neural structures to arthroscopic posterior portals according to knee positioning

Purpose
The purpose of this study was to investigate the relationship between the proximity of neural structures to standard posterior portals in different knee positions.
Methods
Ten fresh cadaveric knees were used to establish the standard posteromedial and posterolateral portals using an outside-in technique with arthroscopic transillumination. The distance from each portal site to the adjacent neurovascular structures (infrapatellar branches of the saphenous nerve) and the sartorial branch of saphenous nerve from the posteromedial portal, and the common peroneal nerve from posterolateral portal was measured using a precision caliper. Distances were recorded with the knees in extension, 45° of flexion and 90° of flexion in order to examine the effect of dynamic knee motion on nerve position.
Results
The mean distance between the posteromedial portal and the superior and inferior branches of infrapatellar branches of the saphenous nerve at full extension was significantly greater than at 90° of flexion. However, there was no difference observed between at 45° and at 90° of flexion. The mean distance from the posteromedial portal to the sartorial branch of saphenous nerve at 90° of flexion was significantly greater (26.1 mm, SD: 4.7) than that at 45° of flexion (18.4 mm, SD: 4.6), and at full extension (14 mm, SD: 4.3) (
P
< 0.0001). The mean distance between the posterolateral portal to the common peroneal nerve at 90° of flexion was also significantly greater (25.4 mm, SD: 9.2) than that at 45° of flexion (22.5 mm, SD: 8.1), and at full extension (20.1 mm, SD: 9.1) (

< 0.0001).
Conclusion
In this cadaveric study, it shows that position of 90° knee flexion is reasonably safe, in order to establish a posterior portal in knee arthroscopy using the technique described in the current study, especially to avoid the sartorial branch of saphenous nerve and the common peroneal nerve injuries. Results of this study suggest the establishment of posterior portals, while the knee is flexed at an angle of 90° is recommended to ensure the safety of these structures.

The Role of Arthroscopy in Athletes with Ulnar Collateral Ligament Injuries

Medial elbow pain is common in the overhead throwing athlete. The diagnosis of medial ulnar collateral ligament (MUCL injuries is mostly based on a history of medial elbow pain, physical exam findings, and imaging studies. The repeated valgus load that causes MUCL attenuation or rupture might also cause ulnar nerve symptoms, posterior impingement, formation of posteromedial osteophytes, formation of loose bodies, stress fractures of the ulna, lateral plica syndrome, trochlea chondromalacia, and less commonly capitellar osteochondritis dissecans (OCD) lesions. Operative treatment of MUCL insufficiency involves open graft reconstruction, but failure to address associated conditions may compromise outcomes of reconstruction. With direct visualization afforded by arthroscopy, the diagnosis and treatment of concomitant pathology may be accomplished at the time of MUCL reconstruction, making elbow arthroscopy a useful adjuvant in the evaluation and treatment of elbow pain in the overhead athlete. The objective of this chapter will be to review the indications and techniques of elbow arthroscopy in athletes with MUCL insufficiency.
