In this study we explored the validity of clinician judgments of voice abnormalities as indicators of penetration-aspiration or other swallowing abnormalities. Voice samples were collected using a high-quality microphone from 40 adults during videofluoroscopy (VFSS), at baseline and following each of four thin liquid swallows. Blinded speech-language pathologists (SLPs) rated the audio recordings for voice quality using the GRBAS scale and the VFSS recordings for abnormal swallow onset, penetration-aspiration, airway closure, and pharyngeal residues. Acoustic measures of % jitter, % shimmer, and signal-to-noise ratio were calculated using two /a/ vowel segments spliced from each voice recording. Preswallow to postswallow measures of voice-quality change were derived and the data were compared to determine the correspondence between perceived voice abnormalities, acoustic voice parameters, and radiographically confirmed swallowing abnormalities. The sensitivity of perceived postswallow changes in voice quality to dysphagia and penetration-aspiration was poor, ranging from 8 to 29%. Specificity was stronger for both penetration-aspiration (75–94%) and dysphagia (59–86%). Acoustic measures of voice quality had moderate sensitivity and specificity for both dysphagia and penetration-aspiration. Overall, perceptual judgments of postswallow wet voice showed the strongest potential for detecting penetration-aspiration (relative risk = 3.24). We conclude that a clear postswallow voice quality provides reasonable evidence that penetration-aspiration and dysphagia are absent. However, observations of abnormal postswallow voice quality can be misleading and are not a valid indication that penetration-aspiration or dysphagia exists.

Malleability and Machines: Glenn Gould and the Technological Self

The Pianist Glenn Gould has often been portrayed as a musical idealist who embraced mundane recording media as a way of escaping the anxiety of the concert hall. In pursuing his musical ideals, however, Gould obsessed over material objects—the qualities of a chair, the action of piano keys, the placement of splices in magnetic tape. This paper argues that for him, the detailed properties of machines and electronic media were crucial, not just as tools for pursuing disembodied aesthetic aims, but as instruments and material sites for a moral project. Provision of taped conversations with neonatologists to mothers of babies in intensive care: randomised controlled trial

Objective To determine whether providing mothers of babies in neonatal intensive care units with audiotapes of their conversations with a neonatologist improves recall of information and psychological wellbeing. Design Randomised, single blinded trial. Setting Neonatal intensive care unit, North Queensland, Australia. Participants 200 mothers of babies in a neonatal intensive care unit. Interventions Mothers given (n=102) or not given (n=98) audiotapes of their conversations with a neonatologist. Main outcome measures Recall of information, attitudes to and use of the tape, satisfaction with conversations, postnatal depression, anxiety, general health, and stress about parenting, at 10 days and four and 12 months. Results 91% (n=93) of mothers in the tape group listened to the tape (once by day 10, twice by four months, and three times by 12 months; range 1-10). At 10 days and four months, mothers in the tape group recalled significantly more information about diagnosis, treatment, and outcome than mothers in the control group. At four months mothers in the tape group were 75% more likely to recall all of the information about treatment than mothers in the control group (59% v 34%; risk ratio 1.75, 95% confidence interval 1.27 to 2.4). Six mothers, all in the control group, could not recall their conversations. No statistically significant differences were found between the groups in satisfaction with conversations (10 days), postnatal depression and anxiety scores (10 days, four and 12 months), and stress about parenting (12 months). Conclusion Providing the mothers of babies in neonatal intensive care units with audiotapes of conversations with a neonatologist enhanced their recall of information (up to four months). The taped conversations did not affect the mothers' wellbeing or satisfaction with the neonatologist. Trial registration Australian Clinical Trials Registry 12606000478516.

A Comparison of the Reliability of Self-Reported Drug Use and Sexual Behaviors Using Computer-Assisted Versus Face-to-Face Interviewing

The purpose of this study was to assess the reliability of responses to HIV risk behavior questions obtained using a voice-enhanced, computer-administered self-interview (audio-CASI) system with touch-screen response compared with those obtained via face-to-face interviews administered by trained and experienced interviewers. Additionally, the study assessed bias that may be attributable to an audio-CASI data collection format. The design of the study was a four-group crossover design with random assignment to one of four study conditions: (a) audio-CASI interview at both intake and retest, (b) face-to-face interview at both intake and retest, (c) audio-CASI interview at intake and face-to-face interview at retest, and (d) face-to-face interview at intake and audio-CASI interview at retest. The study was conducted with a sample of drug users at risk for HIV infection interviewed in nonclinical settings. Data were collected at intake and 48 hours after intake. Analyses show that data obtained using voice-enhanced computer interviewing with touch-screen response are reliable and are comparable to data obtained using interviewer administered face-to-face interviews. However, bias was found to be associated with data collection format and may be parti ally attributable to the complexity of the questionnaire.

Validation of electronic performance and tracking systems EPTS under field conditions

The purpose of this study was to assess the measurement accuracy of the most commonly used tracking technologies in professional team sports (i.e., semi-automatic multiple-camera video technology (VID), radar-based local positioning system (LPS), and global positioning system (GPS)). The position, speed, acceleration and distance measures of each technology were compared against simultaneously recorded measures of a reference system (VICON motion capture system) and quantified by means of the root mean square error RMSE. Fourteen male soccer players (age: 17.4±0.4 years, height: 178.6±4.2 cm, body mass: 70.2±6.2 kg) playing for the U19 Bundesliga team FC Augsburg participated in the study. The test battery comprised a sport-specific course, shuttle runs, and small sided games on an outdoor soccer field. The validity of fundamental spatiotemporal tracking data differed significantly between all tested technologies. In particular, LPS showed higher validity for measuring an athlete's position (23±7 cm) than both VID (56±16 cm) and GPS (96±49 cm). Considering errors of instantaneous speed measures, GPS (0.28±0.07 m⋅s-1) and LPS (0.25±0.06 m⋅s-1) achieved significantly lower error values than VID (0.41±0.08 m⋅s-1). Equivalent accuracy differences were found for instant acceleration values (GPS: 0.67±0.21 m⋅s-2, LPS: 0.68±0.14 m⋅s-2, VID: 0.91±0.19 m⋅s-2). During small-sided games, lowest deviations from reference measures have been found in the total distance category, with errors ranging from 2.2% (GPS) to 2.7% (VID) and 4.0% (LPS). All technologies had in common that the magnitude of the error increased as the speed of the tracking object increased. Especially in performance indicators that might have a high impact on practical decisions, such as distance covered with high speed, we found >40% deviations from the reference system for each of the technologies. Overall, our results revealed significant between-system differences in the validity of tracking data, implying that any comparison of results using different tracking technologies should be done with caution.

Mapping the Early Language Environment Using All-Day Recordings and Automated Analysis

This research provided a first-generation standardization of automated language environment estimates, validated these estimates against standard language assessments, and extended on previous research reporting language behavior differences across socioeconomic groups.
Typically developing children between 2 to 48 months of age completed monthly, daylong recordings in their natural language environments over a span of approximately 6-38 months. The resulting data set contained 3,213 12-hr recordings automatically analyzed by using the Language Environment Analysis (LENA) System to generate estimates of (a) the number of adult words in the child's environment, (b) the amount of caregiver-child interaction, and (c) the frequency of child vocal output.
Child vocalization frequency and turn-taking increased with age, whereas adult word counts were age independent after early infancy. Child vocalization and conversational turn estimates predicted 7%-16% of the variance observed in child language assessment scores. Lower socioeconomic status (SES) children produced fewer vocalizations, engaged in fewer adult-child interactions, and were exposed to fewer daily adult words compared with their higher socioeconomic status peers, but within-group variability was high.
The results offer new insight into the landscape of the early language environment, with clinical implications for identification of children at-risk for impoverished language environments.

Challenging Stereotyping and Bias: A Voice Simulation Study

Stigma is a barrier to mental health care access for patients with schizophrenia and can interfere with developing therapeutic relationships. This study demonstrates success of a voice simulation experience during orientation in changing the biases of nursing students and the effect on the development of the nurse-patient relationship. Ninety-four individuals participated; 52 received a voice simulation experience during orientation, and 42 received orientation with no voice simulation experience. The Medical Condition Regard Scale was administered before and after orientation. Posttest paired t test results show significant differences in attitudes toward patients with voice hearing experiences between the two groups. The themes of personal growth from the focus groups postorientation include Affective Experience, Physical Experience, and Empathy. Findings demonstrate that the orientation process should include methods to challenge stereotyping and bias to decrease stigma, improve service access, and enhance the ability to develop therapeutic relationships.

The Utility of Pitch Elevation in the Evaluation of Oropharyngeal Dysphagia: Preliminary Findings

Purpose: To evaluate the utility of a pitch elevation task in the assessment of oropharyngeal dysphagia. Method: This study was a pilot prospective cohort study including 40 consecutive patients (16 male and 24 female) who were referred by their physician for a swallowing evaluation. Patients were evaluated with a noninstrumental clinical examination and a videofluoroscopic swallow study, and participated in a pitch elevation task during videofluoroscopic image acquisition. Relationships between pitch elevation measurements (acoustic and perceptual) and swallow parameters (penetration/aspiration and residue) were investigated. Results: Results of this pilot study revealed that both maximum fundamental frequency (F[subscript 0]) and perceptual evaluation of pitch elevation independently significantly predicted Penetration-Aspiration Scale scores for thin liquid swallows (p = 0.01 and 0.03, respectively). Vocal range (average pitch to falsetto) was not sensitive in predicting likelihood of oropharyngeal dysphagia. Conclusions: Findings indicate that reduced pitch elevation can be indicative of reduced airway protection and swallowing impairment in some dysphagia patients and may be a useful supplement to dysphagia screening and diagnosis. Further investigation is warranted to determine the optimal utility of this procedure for different diagnostic categories of patients. (Contains 4 figures.)
