تدور رواية الاعتراف حول امرأة تحت الملاحظة في مستشفى للأمراض النفسية ؛ حيث تروي للممرضة الصامتة -في شكل مونولوج- قصة حياتها المليئة بالمفاجئات. كانت ماريانه فيليبس على دراية بالليالي الطويلة في العيادات النفسية، لأنها في عام 1913، بعد ولادة ابنتها الكبرى مكثت في عيادة فاليريوس في أمستردام لمدة ستة أشهر مصابة بذهان النفاس، تحتوي الرواية على مزيج من عناصر السيرة الذاتية للمؤلفة سنوات الفقر. والعمل في ورشة للخياطة، والحياة كامرأة عاملة شابة في غرفة مستأجرة رئة، والأجواء الفاخرة لشركة أزياء، وما يرتبط بذلك كله من أحلام ومخاوف لم يكن من الشائع في عام 1930 أن يتم معالجة التقلبات النفسية المعقدة في الروايات خاصة من قبل النساء، هذا هو السبب الذي جعل \"الاعتراف\" كتابا غير عادي.
