在英語國際化時代回首ESP課程

為因應臺灣觀光醫療產業興起及人口結構改變,護理人員面臨的挑戰也更勝以往,包括醫療專業知識到英語能力,缺一不可。有鑒於此,各類專業醫護英語課程(English for Special Purposes, ESP)也相繼出現,旨在於協助護理人員建構英語相關能力以因應日益變遷的挑戰。然而,相關研究指出,目前護理人員不盡理想的英語溝通能力為一警訊,為臺灣推行醫療國際化及人口結構改變伴隨而來的溝通需求帶來隱憂。教育人員面對此困境時,著實有必要重新檢視ESP課程。本文從三構面探討ESP課程,包含:(1)語言(language descriptions)、(2)需求(needs analysis)及(3)學習理論(learning theories)。筆者回顧與此三構面相關概念與研究,例如:護病溝通特性、醫護社群影響、情境教學、英語教學法(English as a medium of instruction)等,並針對上述等該如何應用於ESP課程,提出個人淺見,以期ESP課程能將學習者需求納入課程設計考量,並將學習者置於教學/學習重心,使課程結合醫療院所情境,幫助護理人員更有自信面對英語挑戰。

背景:護理人員具備良善的溝通能力可減少護病衝突發生。鑒於本單位每月因靜脈注射過程溝通欠佳,平均抱怨件數達20-25件。經調查發現56.25%護理人員自評溝通能力低於整體平均值,故引起專案小組改善動機。本專案透過訪談與實地觀察,發現導致溝通不良的原因為教育訓練課程設計未符合臨床照護需求、人員執行靜脈注射溝通經驗不足及自信度偏低、缺乏溝通實務之影音教材。目的:透過專案設立合宜教學模式,以提升兒科護理人員於靜脈注射過程之溝通能力。解決方案:經專案小組提出改善方案為修訂小兒靜脈注射訓練課程、設計溝通教案、培訓溝通種子教師、溝通實境影片拍攝、定期舉辦案例討論並收錄教戰手冊中。結果:護理人員於靜脈注射過程可運用語言或非語言引導病童及家屬溝通,理解其表達的重點並有條理地做出回應,整體溝通能力由27.25分提升至35.94分(總分8-40分)。結論:本專案結合情境模擬建立溝通教學模式,有效提升護理人員靜脈注射過程之溝通能力,期望藉此專案分享,促使護理人員與病童及家屬建立良善的護病關係。

背景:加護病房(intensive care unit, ICU)護理人員需具備良好的人際溝通能力及適當的衝突處理風格,才能有效的與醫療團隊、病人及家屬溝通。目前台灣缺乏對ICU護理人員人際溝通能力及衝突處理風格相關之探討。目的:探討ICU護理人員的人際溝通能力、衝突處理風格與影響因子,及人際溝通能力、衝突處理風格之關係。方法:採橫斷式、相關性研究法,以方便取樣方式選取南部三家醫院351位ICU護理人員,用「個人基本屬性問卷」、「人際溝通能力量表」和「衝突處理風格量表」收集資料。結果:ICU護理人員:(1)整體人際溝通能力屬中等以上程度;最常使用的衝突處理風格為「整合」。(2)教育程度、職位越高,其人際溝通能力越佳。(3)年齡在「妥協」、「逃避」;教育程度在「讓步」、「妥協」、「逃避」、「整合」;職位在「讓步」、「妥協」、「整合」;工作年資在「支配」、「妥協」、「整合」的衝突處理風格上呈顯著差異。(4)人際溝通能力分別與「支配」、「讓步」、「妥協」及「整合」的衝突處理風格有顯著正相關。結論/實務應用:護理管理者為ICU護理人員舉辦年度培訓課程時,可納入人際溝通、衝突處理案例討論,使之瞭解各種人際衝突處理方式的利弊;並建議未來研究者可針對ICU護理人員了解他/她們對自己衝突處理風格的看法。

Background: Clear directions and explanations from nurses related to health behaviors and discharge procedures have been shown to effectively reduce the risk of patient readmission.
