Overview
Blood tests showed hypercalcaemia (2·73 mmol/L; normal 2·10–2·60) and elevated angiotensin converting enzyme (139·4 U/L; normal <79·1) and creatinine (224 μmol/L; normal 45–100), suggesting sarcoidosis. Given impaired renal function and rapidly progressive disease, a shared decision was made to urgently begin chemotherapy with etoposide (100 mg/m2, 5 doses) and carboplatin (AUC 4, 1 dose), with acknowledgement that carboplatin is an inferior chemotherapy drug in this disease state. Granulomatous reactions, sometimes with elevated angiotensin converting enzyme and hypercalcaemia, have been reported with various malignancies.
