Overview
In the USA at the beginning of the 20th century the infant mortality rate was high (over 150 per 1000 livebirths) and 19% of deaths were estimated to be due to pneumonia, a similar proportion to that in developing countries today.8 At that time there was a major political movement in the USA, driven by concerned health professionals, to deal with the problem of infant and child mortality.9 This movement focused on improved nutrition, but it coincided with a period when living conditions were improving greatly. In the near future we might see communities in poor countries in which the introduction of pneumococcal and rotavirus vaccines has been achieved at the cost of contracting case management services, so that children who once had access to basic antibiotics and oral rehydration solution for treatment of pneumonia and diarrhoea only have access to these expensive vaccines.
